Hi dazed -
I'm so glad you're moving on from the first LLMD. You need a partner you are confident in and trust. For some of us, this takes a few attempts but you have a good "take charge" attitude, which you're going to need! There are EXCELLENT LLMDs out there - you just gotta find the right one.
Believe it or not, many people would love to have an LLMD within an hour's drive. Perhaps there are some viable options you can consider in order to get to your first couple of appointments (have a good friend drive you, hire someone, etc.), and then maybe the LLMD would be willing to do phone consults with you every now and then afterwards. It's worth a shot.
Yes, you will likely receive a dx of late-stage or chronic Lyme. Again, a solid LLMD will be able to dx w/out testing. Your insurance company might require specific tests and specific results so that is something to prepare for. Most LLMDs don't participate in insurance plans but it's still possible to get your treatment covered.
Quite honestly, if you read through the "New to Lyme?" thread at the top of the page, you'll understand how your test already confirms being positive for Lyme. The proof is already there - you just need the Cliff Notes for how to interpret the test, which is in that thread.
Even if your test was negative, which it isn't, the CDC recommends that the tests not be used solely for diagnosis - that MDs should also consider exposure and symptoms.
Some LLMDs and insurance companies still like having a CD57, which is inexpensive and is supposed to indicate if your immune system is struggling in a way that was once assumed to be unique for a condition like Lyme. But more and more, this test is not providing the kind of information that is helpful with diagnosis or treatment.
Your sx are in line w/ many common Lyme sx but most people who have Lyme also have other co-infections, which are carried by the ticks as well and can include similar symptoms. Here is a very helpful questionnaire you might want to fill out and share w/ your next LLMD - very helpful for diagnosis & treatment:

You'll want to have a solid detox protocol in place before starting treatment, anyway.
