You've been given really good info already. Particularly detox suggestions. For the headaches AND brain fog - please try Nutrimedix bur bur and Pinella tinctures - they're essential for my brain detox. A lot of us are using them.
Also for brain detox - binders are helpful (remember the gut-brain connection) so I like to do a binder as the last thing I do for the day so that there is nothing for the gut to absorb overnight that shouldn't be there.
I also wanted to suggest trying to find a Lyme-literate neurologist to consult about
the seizures, etc. I have one in San Diego - love him but that might not be convenient for you. He likes to suggest that most neuro symptoms are caused by inflammation so that is something to try (japanese knotweed, turmeric, maybe do a search for other remedies).
I started seeing him when I developed seizures, vertigo, tinnitus about
1 yr into antibiotics. Doing much better now (he didn't do a whole lot for me other than run a slew of tests but he did give me a recipe for tinnitus cocktail:
Niacin 100mg once/day
L-lysine 500mg once/day
Lipoflavonoid one tablet once/day
