There are just as many ways to take herbs as there are herbalists. Each one, I have found, have different takes on it.
The majority say, to take a tincture drops directly under the tongue. This is with a clean mouth, meaning not having just eaten or having had anything to drink, especially coffee.
Now this is not going to work for us that take 5-10 or even more tinctures. Plus the taste of some of those herbs taste horrible!!!
So it is totally fine to put the tincture drops in water or juice. Or some herbalists say that just a drop of stevia or a dab of honey with the tincture is acceptable.
For me personally, I did the water route. All my drops went into a high ball glass (showing my age here), then I topped it off with water and chugged the sucker down. I then did a shiver because of the taste.
The reasoning for holding it in your mouth is the absorption is quicker and greater to the body by holding it in your mouth. But, even with swallowing it, some remains in your mouth.
I don't think this is something you should worry about. As long as you are doing your herbs regularly and consistently, you are treating the disease.
The more important question is -- how are you doing? Is this working for you? If it is comfortable and working for you, don't make a change. We all find our own way in the treatment of lyme and NO ONE has a perfect, correct way. All we do on this forum is give past experience. You have to be your own guinea pig. But, like I said, it is working, why change.
Good luck.
