It looks like it is still lyme. Lyme does change, the way you describe, specially after you start antimicrobials such as doxy.
I hope you have a good doctor, but I would not wait to continue treatment. Lyme is usually clinically diagnosed. Serology will only support the diagnosis...
Most people are infected with the 3 Bs: bartonella, babesia and borrelia. Treating them would be a good start, in my opinion.
Anxiety is extremely common. Too common with infections and with lyme.
I would start on binders, such as MSM, chlolestyramine, chlorella, apple pectin, stuff like that, to help you lower toxins (or nightmares and night terrors may increase...).
amoxy works a bit for Bb, but I think it works for no coinfection (I can't remember any coinfection that reacts to amoxi)...
If your gut is a mess, go for probiotics and enzyme treatments, plus L-glutamine (I'm trying this last item recently, it feels wonderful).
If you are newly bitten, antibiotics may give you some positive outcomes (short term antibiotics).
but if you are long term infected, they won't probably work, according to dr. K (who treats lyme since a very long time; he has been working with lyme since he left the university, it seems, back in the end of 70s, beginning of 80s)
but there are so many other alternatives to treat lyme, such as herbs, microcurrents, rife devices, bee venom, biophotons, laser, EAV type of treatments, etc...
I hope you find your way out soon. Buy Buhner's books, they are quite inexpensive, and are very much worth for empowerment.
