Hi everyone,

I just received my results and was hoping for some assistance since everyone on the board is so knowledgable, and my doctor was unable to speak specifically to which bands were specific to lyme or not.


IgM
IGX: Indeterminate
CDC:Negative

31+
83-93 I

IgG
IGX: Negative
CDC: Negative

31 I
41++
45+
58+


My doctor recommended the 31 dka epitope confirmation Igenex test, which I agreed to. From the little I've gathered, it appears this may be late lyme?

The epitope test will determine if band 31 is indeed from Borrelia...so for the $100 it's worth it in your case IMO.

Although, according to Dr. H. it's rarely something else.

Unfortunately with Lyme, it's difficult to determine early vs late infection.

IgM means active infection, but can still be an older infection.
IgG means older infection, but can still be active.

I was postive IgM through Igenex - and this was a 14 month old infection. (That's how long I had symptoms...anyway)

Thanks both for the replies. I will go ahead with the epitome test.

I called igenex for a bit more clarity, and she said that my IgM also has band 41 positive, but it reads negative on the print out I got from my doctor. Wouldn't that make it positive for lyme??

Nevermind, sorry, it sounds like bands 31 and 41 are the exception. Clearly a newbie here.

If you do have band 31 and 41 - if the epitope test confirms band 31 is Borrelia, that will change it from Indeterminate to Positive result.
For your Igm to be Ind you must have also had band 41 +

It doesn't say that on your report? Do you have the igenex result sheet - or just a different report from your Dr.?

I have a print out from my doctor and band 41 is negative on my sheet, but looking at it closely, it looks like a printer error and should have been positive (confirmed by Igenex).

Then that epitope test can (if positive) change your IND to positive.

I've only heard of it not being a positive once so far.

And if it's negative? Do you think an LLMD would proceed with treatment anyway, or are you aware of a differential diagnosis?

Well you have an IND on band 83-93 as well - which indicates a response...just not strong enough for it to be a positive.

A LLMD may still proceed with treating you based on a clinical diagnosis....
(did you have a known tick bite and/or EM?)

Or may ask for more testing to be done.

It depends on the LL Dr.

Thanks, Girlie. Unfortunately, my doctor isn't an LLMD, and there aren't any in my city, so may have to take some herbs to provoke a herx and then retest.

I don't remember any tick bite or an EM, but I do recall developing a large bruise like welt on my knee after being outside.

Her Dr isn't LL - so doubtful if cd57 would be accepted as a test to determine lyme.

Even some LL Dr's don't use it (mine doesn't)

Funny how it is good to get that confirmation. I found an ILADS trained ND who will work with my insurance (I have to pay my copay and a clinical fee), but my appointment isn't until March. I'm trying to have my current doctor send a referral with the hopes of getting in sooner.

The epitope confirmation test seems pretty conclusive, do you know if the CDC/traditional doctors recognize it or treat it as a positive?

Doubt it - CDC has its own criteria:

"The Lyme IgM Western Blot is considered positive if 2 of the following 3 bands are present: 23-25, 39, and 41kDa."

Hi Girlie,

I know that you mentioned positive on the epitope test will result in a positive IgM. Igenex only sent me a positive on the epitope but did not update my IgM, is that something I need to request from them?

That is all they send. They won't update the previous one , they just give you the second sheet saying it's positive...it means that the 31 is Borrelia....and it's not IND result anymore.

Just staple the epitope test result with the other one.

A LLMD should recognize it...and know what it means.
