Hey all,

As some may know I've been dealing with chronic Q Fever (which is a Lyme co-infection) for about 3.5 years now. I'm currently on ABX treatment for it, but to be honest I'm not sure if it is the best treatment I could be on, in part because I haven't been paying attention to the new protocols for Lyme and co-infections. So, if anyone has come across effective protocols for it, I'd love to see them.

Just for fun I'm currently on Doxy 100mg 2x, Plaquenil 200mg 3x and Biaxin 500mg 1x. FYI doc is taking me off Plaquenil and is putting me on Flagyl 250mg 2x.

What I attribute as my main Q Fever symptom (pulse pounding in my head) has gone completely away, although I still have a boatload of other symptoms, which I attribute to mycotoxin issues, but am not sure what role Q is playing. But as I can't completely eliminate mycotoxin exposure due to work, I am trying to still hit Q. Doc also thinks I have Bart, so that is why the Biaxin is in there.

Would the new Lyme protocol with Dapsone help perhaps?

I don’t know anything about Q Fever to say whether the Persister meds would help - but I can say that the Persister meds are strong!
So it should at least hit other infections that you have lurking in the background ...

Dapsone/daraprim/Rifabutin - those are hitting me hard - and I’m not sure if it’s one in particular or the combo of all three.

Thanks Girlie! Are you on lots of supplements to mitigate the side effects of dapsone and the others? If so, what? Where would my doc (or I) find the protocol info?

I don't think there's anywhere to find the protocol I'm on. It's ala Dr. J.

I take Folic Acid with Dapsone and daraprim. I think it's the Dapsone that blocks folic acid synthesis. I had my level tested prior to starting the protocol and take folic acid with every dose.

Other than that - not on anything specific for that protocol.

Maybe there's some info on the Dapsone - in this study (ignore the reference to 'post treatment lyme disease syndrome')

Chronic Q fever is very very diferent to lyme... because it's symptoms are not inespecific like lyme. It's even diagnosed in a normal hospital. I know that because my girlfriend is veterinarian and some partners get coxiella burnetti from a dead animal and all were cured without relapses...

Are you considering Bart or lyme for your symptoms? Maybe it's the best explanation .

Have you done all the functional medicine stuff before this protocol? fix gut. treat heavy metal. parasites and remove dental infections and root canals.

I'm not sure how they differ. They're both in the same class but Rifabutin seems much stronger than Rifampin.

OriolCarol - Thanks for the question. I've been diagnosed by two separate LLMDs who are familiar with the disease and blood testing confirms it. My symptoms are mostly mycotoxin related at this point, but as Q Fever can cause extensive damage to the heart (endocarditis) efforts are ongoing via treatment to keep me from developing it by trying to get the numbers down. Q is also considered a co-infection by Horowitz. I did see a normal hospital infectious disease doctor and got the same ol' runaround as I did when I thought my mycotoxin symptoms were Lyme and that was despite me having multiple positive blood tests.

Magoo - thanks. Yes, my LLMD is functional - and working on the dental stuff now.

Girlie - had a good discussion with my doc today and she says based on the literature Dapsone can be helpful with Q and she's willing to try me on it.

Do you think you will do the Dapsone, Mike?

Will you have it combined with another antibiotic? or did you not get into details with your Dr. about it.

Is dapsone effective to treat Q fever? You could try tetracycline 500 mg 4x day plus hydroxychloroquine (if you have q fever, but i don't like it.. Plaquenil has a lot of side effects and can be long lasting...)

Girlie - I think I will, but may wait a little bit as I'm having some lymph issues I want to deal with first. She suggested this protocol based on Horowitz's work:

Doxycycline 100mg 2 x daily
Rifampin 150mg 2 x daily
Dapsone starting at 25mg daily for the first week increasing to 50mg as tolerated. Horowitz continues to go up from there but it is a good place to start.

I don't want to be negative but this protocol will give you a higher rate of failure in any scenario...

There are a lot of reports in medical literature about faillure of doxy/rifampin to treat coxiella burnetti... The reason is that rifampin decrease 40% doxy... so in fact you're taking less than 120 mg of doxycycline and the dose of rifampin is too low to treat q fever... it can be extrapollated to lyme.. 120 mg of doxy will make things worse...

Why not 200 mg minocin plus 600 mg of rifampin? Minocin is less afected by rifampin.

I appreciate your candor and knowledge as you seem to understand Coxiella. Mino has given me lots of gut pain, which the other abx don't. Do you have the literature you can send me? Thanks!
