I had the same thing happen with bart. Initially negative lab tests and few symptoms. Then I took antibiotics, and I had a huge bart infection and a positive PCR test.
There are a few possible reasons for this. One is that when borrelia creates biofilms, other bacteria can hide in there. When you break up the biofilms, it releases the other bacteria, including bartonella and babesia. See this thread

The other thing is that bartonella can hide inside cells and remain dormant for awhile, but when it gets hungry, it comes out and attacks red blood cells. I found that taking certain antibiotics would not kill it but would stress it enough that it would come out.
How long have you been taking disulfiram? Disulfiram has a slow elimination and accumulates over time. People sometimes report that they feel nothing at first, but after a few weeks get a strong herx. Disulfiram is reported to be effective against borrelia and babesia, but it's unclear if it does anything to bartonella.
