Diarrheas from nystatin and probiotics - why?

I will soon start abx treatment after being off for 3 years. To prepare I wanted to start probiotics and nystatin... but from both I get diarrhea and I don't know why.

First I tried probiotics, Innate ultra flora, and when increasing to 20billions a day I got diarrheas which stopped after stopping the probiotic. After that I tried nystatin pills and also that gave me diarrhea. I have taken both last time I took abx without any issue.

Nystatin will kill gut flora and give you diarrhea. After you poop out whatever you were trying to get rid of, take probiotics to put better bacteria in your gut. Isn't that the whole point?

Nystatin kills gut flora? It’s supposed to kill yeast. You have references to it killing normal, desirable gut bacteria?

It kills yeast, but there are normally some yeast in your gut. So the yeast die, and you get diarrhea.

@Quin: The idea was to start nystatin before starting abx so it could help me to not get yeastproblems when taking abx... and also to start probiotics due to the same reason, but after I had been taking nystatin some days I got diarrhea - not several times a day but watery (sorry..). I stopped nystatin and it got better again. Then I started probiotic and my stomach was fine... until I increased the dose to 20billions and also took megasporebiotic... then this diarrhea started again.

Do you mean that it is normal to get diarrheas from yeast die off? Maybe this is also the cause to why I get diarrheas from increasing probiotics... and in that case I guess I will probably tolerate more probiotics when starting abx because then it needs higher doses to kill candida.

I feel a bit stupid but for some reason this makes me worried now when I am about to start abx (a lot of things worries me right now... sad ). It doesn't feel good to start abx when having diarrheas but maybe that is ok...

In theory, the bacteria in megasporebiotic shouldn't be affected by nystatin, but who knows.

You're trying to treat candida with nystatin? Nystatin isn't absorbed from the gut, so usually you'd want something like fluconazole for that.

@Quin: Do you mean that the killing of the gut flora from nystatin is what is causing my diarrheas? But I have gotten diahrreas from nystatin and probiotics seperate... not taking them all at once. First I took nystatin and got diarrheas after some days. I stopped and it got better. After that I took probiotics which worked fine until I upped the dose and the diarrheas started again.

No my thought was not to treat candida with nystatin because when I started I didn't feel any candida problems . I asked my Dr if I could take fluconazole during my abx treatment to avoid candida problems but since I am going to take azithro he wouldn't prescribe fluconazole at the same time because of both affecting QT interval. Because of this I started nystatin pills to get some help during abx treatment.. but as I wrote that gave me diarrheas and I stopped.

Now after I have been trying nystatin, increased doses of probiotics and albicansan (Homeopathic from Sanum Kehlbeck to treat candida) - not at the same time - I have gotten (except for the diarrheas) itchiness down there, nauseous and a bit white tongue... which for me sounds like candida die off so eventhough not feeling any candida issues when starting this I seem to have some.

I lean towards that my diarrheas also are a candida die off symptom but do you know if diahrreas is common as a candida die off symptom?

the gut, the immune system which is in contact with it through the gut membrane and the gut microbiome are a very complex and finely balanced system. pretty much any chemical you put in there can often upset that balance in some way - and it will then take time for it to find a new balance.

there are both hundreds or thousands of different bacteria in there - and commensal fungal species and viral species also - even a so called anti-fungal may well have some off target antibacterial or even anti-viral effects ( there are at least 10x as many viral particles in the average human gut as bacteria ) - or just irritate or stimulate the gut in some way and we simply do not know enough yet to predict or understand these effects fully.

but in essence - just because a drug says its an anti-fungal - and at the very top level is not expected to have major impact on the microbiome - that is typically a gross oversimplification - and doesn't mean you will not have loose stools as a side effect. in fact its top of the list of side effects in teh drug info leaflet ...

Common Side effects of Nystatin include:
Diarrhea.
Nausea.
Stomach pain or upset.
Vomiting.
Contact dermatitis.
Stevens-Johnson syndrome.
Hypersensitivity reactions.
Skin irritation or redness.

Any anti fungals and probiotics break my biofilm. Fluconazole is known to break biofilm. Perhaps nystatin also does this and thus worsened your condition (hopefully temporarily).

Bailey, I had a significant reaction to Fluconazole and do think it broke biofilm, though I seem to do okay with probiotics.

@Garzie: Thank you for your thoughts! I agree with you! I totally missed that nystatin has diarrhea as a side effect. I however didn't get that the first time I took nystatin but I guess that could change. I get confused about how to handle higher doses of probiotics when starting abx.. hopefully I will tolerate more then without getting diarrheas.

@Bailey: Yes I have heard that fluconazole is a biofilm buster but have never experienced that from probiotics.

I’m super sensitive but especially when is tart a new probiotic it’s more pronounced. I’m the only person I know that has this issue with probiotics, though my doc actually didn’t look at me sideways when I said probiotics break my biofilm like he’s alt to do with many of my other reactions... so apparently on the list of weird ways my body reacts, he didn’t think it was super weird.

@Bailey: I am also supersensitive to a lot... that is really frustrating! Do you continue taking probiotics eventhough having those issues?
