J-Pouch 2-stage surgery question.........I should have asked the Doc for clarification

Having j pouch surgery as soon as surgery time is available.
If I heard surgeon correctly......... step 1 he will remove my large colon, make the j pouch from small bowel........connect it to the rectum and do temporary ileostomy. Temporary diversion allows j pouch and connection at short rectum to heal completely

Two questions
How does surgeon construct the diversion apparatus to completely seal off the small bowel?
Step 2 - ileostomy is removed and closed. Is there not some repairs done at the small bowel end of ileostomy? This area would not have chance to heal as BM stuff has to go somewhere

Well, I'm not a surgeon, but from the way I understood it....when you have a temp ileo it usually a loop. So it's like a garden hose, they fold it, stick the fold through you abdominal wall, and make an opening there. So, most waste goes out the hole. It is not a complete "Seal", a tiny bit of stool will sometimes get through, and you just pass that and mucus like "normal". After the ileosotmy is taken down, you go through your bum like you used too. I am sure that BM stuff is flowing over stitches and such, but it's just the way it's done, I guess. Everything heals up nicley anyway.

i don't think it would be any different then when you have a resection...they have to stitch the bowel back together and it always seems to work!! Plus, I think the surgery can even be done in one step, so the stitches/staples should be enough to keep everything together:) Good luck with your upcoming surgery, keep us posted on how you are doing!

The jpouch surgery involves removing both the colon and the rectum.  The first surgery removes both organs, creates the jpouch and connects it to the remaining rectal cuff.  The ileostomy is like a hose cut in half so you will have some diversion from the small intestine to the new pouch.  Don't be frightened if your new pouch starts functioning before your ileostomy is closed.  It's normal.
