Need your insights on new ileo stoma

my new stoma from the az mayo doc protrudes an inch out and is really easy to pouch.  As you know she took out my unused colon which was a surprise to me as she was worreid about the heavy mucous accumulationn in the cecum at surgery and as long as she had me opened, she took it out, tho this was not part of the plan.

so my ileo stoma looks great, but it is now leaning to the right a bit. not retraction and the opening is still in the center but i am wondering is the slight tilt to the right hampering its ability to expel stool, or seeing as i have quite a bit of pain up under my right rib cage if I lie on my left at night, does is sound like an adhesion is forming from where she took the colon down and could that be hampering the function of the new ileo stoma.

I am outputting now a short while after eating breakfast and actually the stoma spit out an undissolved pill from last night (no wonder I did not get pain reliefl, it did not dissolve.)

so let me know about anyone's stoma that comes out an inch and does yours lean to the right or left?

also, due to not eating at night because i cannot sleep if food is in me while, I sleep, is what  you all put out in the morning what you just ate or is it what you had the night before?

it has been so long since i had an stoma work normally that i lose total track.  Thx.  Rosemary

Hello Rosemary, I'm glad to hear a much less urgen problem, although I don't see a problem here. I only know the the end and loop stomas that I had, plus the photos I've seen on the web and I have seen some pointing a bit to the right or left. I can't think that that would cause any problems.
The small intestine always has some amount of matter in it, so that what comes out in the morning or any time could have been there for a while. I know you have been watching for everything you ate to come out and connect it to something specific you ate at a specific time, but from my experience it doesn't necessarily go in that order. My noon bowel could look like my breakfast, but the bowel after that one could still have bits of the vegetables I ate the night - or even afternoon - from the day before. I have no explanation for that, I just think, good, it's coming out.
With those pills, I stopped taking them in pill form since I had my colon out. I have no way of knowing if it's going to get taken up or land in the toilet. So if I've had to take antiobiotics I get the liquid form. The only pills I take now are immodium, which definitely get taken up, and celebrex capsules for knee pain, which my GI suggested and said they would take up. Ask your doctor if you can break a pill in half so that it dissolves earlier, or for another form that will do you some good.

My ileo points a bit to the right and down. I just accept it as is although I have to admit that the pointing down part is new to me. The change has me a bit weirded out just because it's new.

As for my food..I find it mixes as well and comes out in strange order sometimes. Late this morning I saw pieces of lettuce (ate yesterday around 2:00 pm). I also saw that last night. But I also ate tomato soup last night and had that in my output last night as well (before the lettuce). I could smell it, haha.

As for pills..I have pill chunks come out quite often. I'm visiting an ostomy nurse every two weeks to take care of pyoderma. Last time she asked "what medication are you taking" There were little granuales in my output that resulted from the pentasa I'm taking and she saw them. So I'm guessing that seeing pill remnants in output is quite common. It doesn't bode well for pain relief though :(.

Yep you are right. Seems like liquid pain killer would be best! Interesting about the racing foods. Interesting that your stoma goes to the right and down. Mine goes down too. I so pray that I am able to eat more or even if I have to live liquidy diet as long as this keeps working enough. I am totally at the end of my rope with surgery and scar tissue etc. Glad I am doing all the site seeing I am. If all stays "well" tomorrow I head to sedona for a day trip. Thx for input on my questions. R

     Don't take the Enteric coated pills.  I don't think they dissolve in the stomach.   I usually take the capsules or liquid forms.  As for the leaning of the stoma, I think mine is pretty much upright.  From what I can observe, the fecal matter comes right out from the top, like Vesuvius shocked .  Sounds like you are doing a LOT better....great to hear this.

I am sure that you know that with an ileo you cannot take extended release pills. The extended release pills do not dissolve fully until in the colon. And guess what.........there ain't one. Part of the pill goes out the stoma.

Hi Dan and Christine,

Yes the pill I took wasn't time released or enteric coated just normal coating that dissolves in water (that's how I test pills I take) but thx for reminders.

The site seeing is saving what little is left of my sanity :-) this was my surgeons idea! It went from 90 to under 60 degrees In just a couple days. Had to go buy some winter stuff. R

This is sort of unrelated to what you are actually asking, but I noticed in this post as well as in some of your other posts that you mentioned having pain when you lay on your right side at night. I'm not sure exactly what kind of pain you mean, but I could not lay on my right side after any of my surgeries without pain. It took a couple of months for that to go away completely. I don't know exactly why it happened, except that my body was just unhappy after having been cut open and messed around with. Just thought I would mention that, perhaps it will help give you some peace of mind it.

Enjoy Sedona! I was there a couple of summers ago and it is gorgeous.

Thx actually it is my left (non stoma side) I can't lie on but thx anyway for the info. Hoping all stays "well" so I can do sedona as planned. Really cold here now wow!

Rosemary,

My stoma sticks out almost 1.5 inches and definitely points down and to the right. The plastic of my pouch sort of pushes on it too so that it flops downward even more. When I am changing and there is nothing over my stoma, it sticks out more straight. The fact that it points down causes me no problems. If anything, it sends output towards the bottom of my pouch. At first when I wore ostomy support wraps or briefs or tighter jeans, my stoma looked like it was getting smashed which freaked me out. However my nurse and surgeon said this was no big deal and it hasn't been. Everything comes out just fine and my stoma never bruises or anything.

As far as output overnight... I eat dinner anytime between 5 p.m. and 9 p.m. depending on my work schedule, evening exercise plans etc. No matter whether I eat on the early or late side of things, I am always still outputting the previous night's dinner until 10 or 11 a.m. During the rest of the day, it seems like things go through faster. I really don't know why this is. Maybe digestion slows down when sleeping? I don't have any pain though. Sometimes I can feel my stoma outputting with a tiny bit of pressure, but it isn't painful.

Thx for input. My pain isn't from the stoma it is under my right rib cage but is seeming just a bit better. Our guts do slow when we sleep so seems like it makes sense stuff you output in day goes faster. Just yesterday and tonight what litte I ate seems to be traveling faster so that is a good thing.

Hi Rosemary, thanks for asking. My bm's are very reduced, down to 6-8 per day, and I get up once in the night, though I often find myself sleeping blissfully through the night too. Last night, however, I had to get up 4 or 5 times and cannot relate this to anything I ate. (OK, I did have two large cookies and some ben and jerries but lots of sweeties haven't caused additional bm's in the past).
Anyway, I want to suggest you turn away from your stoma, your digestion and what you are eating, to other things, for hours at a stretch. I know you like to travel, but I mean something that you enjoy that doesn't take lots of planning. Something you can just pick up and concentrate on for hours. Gardening is out for a few months. Resarching something you think is exciting (I don't know what. I could read for hours about obscure film noir, you must have some things that you hunger for info about). Get my drift? Stop living with your head inside your stoma. I do not mean to sound rough and tough, but it sounds like you are doing relatively alright enough that you need to put your head into a very different place.

I have to agree with what b'dereh just said, stop thinking about your poop! And your stoma! They all look different, if it works it doesn't matter! All our stuff comes out t different times, and in different form and it doesn't always make sense how it comes out. You seem to be doing good right now, hopefully you are eating real food and not baby food and crackers and yogurt all the time!
Get out there and travel and eat a greasy burger, spend all day shopping. Hang out with your friends at a bar or resturant. Go on another cruise! Find something to keep youfrom thinking about your poop all the time:-)
