Just got home from ER :( I do have my 7th raging UTI. I'm also down 14 lbs since April 1st from nausea/vomiting and that was with sweats, shirt, sweatshirt, and tennis shoes on :(My urine was nasty...positive for: WBC's >50,RBC's, blood, bacteria 2+, ketones 2+, nitrites, and leukocyte estrase. Did chest X-ray, and X-ray and both ok. Labs showed dehydration...Albumin high showing chronic and severe dehydration. Liver enzymes and Alk phos elevated. Besides slightly low potassium others ok.

Could not get IV started...six pokes and finally in thumb. Told me I need port or PICC again. Gave me fluids, toradol, IV dilaudid, zofran, and phenergan. The ER doctor said 'I'm just a dumb ER dr and your way too complex.' He has no idea why I'm vomiting. He thought I had obstruction, but X-ray ok. Asked about Cdiff and he said he didn't think would be causing this much discomfort, vomiting, consistent nausea, and wt loss?!?! I don't know enough about it so your opinion is greatly appreciated. Just tried sherbet as I thought with meds I would be ok?!? Told him foul foul odor and excessive gas as well?! Anyways, the sherbet was NOT a good idea. Crouched in ball and will most likely not keep it down. Any advice?

I am still awake. Ugh! Sorry to read your post. Praying for us both. What is the big deal about thm not ruling out cdiff? That was an easy test they did at CC for me last week or two weeks ago. I lose track. Good luck. Hope you get some sleep. I just want to sleep too. RJ

My local colorectal surgeon did order a CDiff check and I had that done other other labs today. How long does it usually take to come back? His nurse said may have to scan brain to r/o any issues with brain that could be causing symptoms. I tried a vanilla cone from McDonald's and only made it 10 minutes before vomiting :( reviewed my labs today...potassium borderline (I take supplements), all liver functions elevated, Albumin and protein levels abnormally high (mine usually low d/t malnutrition and this is sign of dehydration, lipase low which is sign of high BP, swollen bladder, and malabsorption of vitamin and minerals (my vit D and b12 in tank), and then dirty urine. He is trying new antibiotic as he thinks may have resistance to Cipro. If its not Cdiff are there any other suggestions you can think of?

The Cdiff result is fast. I got mine online the next day. Mine was neg. but as you know I have other issues. For the sibo (small intestinal bacterial overgrowth) on gi gave me cephalexin that gave me bad diarrhea. Then tried flagyl that made me so dizzy I could not stand up. He finally said take a probiotic. As you have to kill off the bad bacteria first THEN maintain with a probiotic I am not so sure the logic for that, but whatever.

The gi who tried to dilate the stoma with the balloon said take Xifaxan. Then did not give me an RX for it. Oh well. MAYBE and this is total guess, did they mention SIBO to you? Likely not but I know by night I am severely distended, but in my case it could be backed up liquid poo behind the stenosed stoma God only knows.

Maybe others will be along with more guesses. I am not sure my intestine even if a new stoma can be made on the other side would even remember how to "chew" real food! My last meal was Nov. 21st and that was yogurt that got stuck. IDK. WE NEED A TEAM of the world's best docs and I still think they could not sort this.

lizzie- really quick as its late for me and i just had a very long day at Cleveland Clinic myself.
cdiff trest they should have in one day. h ound them for the results if need be.

as for sibo, standard first line of treatment if rifaxamin. Its a realatively harmless antibiotics as they go.. mostly aborbed in intestinal tract so reduces all the side effects from cipro, flagyl, etc. Very exepsnive so make sure your insurance covers it.

wondering if cdiff is negative that you maybe have partial obsturction of sorts?

also you mentioned short gut, so some of your labs being so far off can be from that. But SIBo also can cause malabsorbtion.

and also oral vanco for cdiff.

also with short gut, not supriseing gyour vit D and B12 is "in the tank" Someone way earlier should've been watching you for this and supplementing. I give myself weekly B12 shots. and intake large volumes of Vit D.

I've had an awful night and actually am still up with no sleep and just miserable. After the vanilla cone I tried 5 saltines and maybe 1/2 c white rice. I was nauseated while eating and tried to hold it in as long as I could, but had forceful vomiting about 30-40 min after. My throat hurt so bad from all undigested rice so I tried some sherbet to cool throat and immediately was curled in ball in so much pain and vomited about 15 min after eating very small amount. I'm better off not eating as its much less stress in my body. I will look online tomorrow for Cdiff results. I am not sure about SIBO? How do they check for that? I do have a functional short gut...I had severe dumping syndrome (no colon, no rectum, 2failed kpouches from small bowel and then two new ileostomies and relocation) at first and lived in hospital and on TPN and after dropping to 87 lbs I had nose tube and then PG-J tube and slowly introduced food, but then the edema got so bad and all the new symptoms started. Nobody had been monitoring my Vit D or B12...my hormone dr decided to draw d/t my chronic fatigue. She wants me to triple the Vit D and gave me sublingual B12 and said may need to switch to shots d/t absorption issues. I'm just getting nervous as I've not had nausea/vomiting/dehydration like this in such a long time. I was doing relatively well with food for first time in 5 years and had finally advanced past soft/low residue diet. Does anyone know much about CIP and pseudoobstructions? I appreciate all your kind responses and time you take to respond. ddd...I hope you had a good day at CC!!

lizzie until they get to the bottom of this. maybe try liquid only diet. Anything you want to eat, but just put it in the blender. Also are you drinking a proper oral rehydraton solution beverage. Your dehydratioin can really be whacking with other things and causing further nausea, etc.
Whey didn't they admit you if so many things were wrong. Did they give you enough iv hydration why you were there.
I take weekly B12 shots, without that, I have more strange problems going on. When you lose the bottom of your small intestine, you lost B12 absorbing ability.
I'd definitely stay far away from anything at all with sugar. That will increase output dramatically, which you surely don't need if you are dehydrated.

Thank you ddd for your reply!!! Today has been a long and exhausting day. I haven't eaten, but drank some, def not enough. I am just so nauseated and everything sounds horrible. I have a massive headache and can barely see straight. I have only emptied a half of bag all day...just bile. My Cdiff came back this afternoon and its negative. I only had one bag of fluids because they could barely get an IV in so I had a small 22G on the bone in my thumb and it took hours to get in :( I really need some sort of access, but PICC's have been a nightmare with DVT's and my right arm cannot be used anymore for PICC lines.

My internal med/hormone dr and cardiologist collaborated regarding all this crazy endocrine stuff and I'm having further imagery, but I'm just not sure what yet?! In ER my Bun/Creatnine were stable...I've only had issues with pre-renal failure once, but I got a bmp drawn yesterday and my bun ok, but Creatnine jumped to 1.07 if I recall from 0.84 and its been slowly trending up (kidney function) but hasn't been this high except for Mexico when it was even higher. My potassium was 3.3 so slightly low, but I can try and supplement that. I'm thinking of asking for the B12 shots instead since I'm sure it contributes to how cruddy I feel :( I don't really know what to think or where to go from hear regarding this intractable nausea and vomiting. If something doesn't change, I will be back on TPN eventually and I don't want that. I honestly don't think I could even do liquid food :( just water makes me nauseated. Laying down to see if head will stop pounding. Any advice GLADLY accepted.

Should I go back to ER? I don't know what to do and I don't see my colorectal surgeon until May 15? After the negative Cdiff and throwing up three times on Tues from eating, I haven't been able to eat/drink (barely) anything. I am nauseated from the time my eyes open until the time they close, which isn't very often. I weighed myself last night and I'm down now 20 lbs since April 1st and 5 lbs since ER on Monday. I tried some sprite and water yesterday as I 6spent entire day at hospital as my mom had surgery on her cervical spine. I was in tears all evening and night d/t the nausea never letting up. I haven't been like this since my constant obstructions, but I haven't eaten and X-ray on Monday didn't show obstruction. My heart rate resting is 119 or more and after sleeping just a couple hrs I am up and the nausea is just as bad, if not worse. I called my pcp (he's been disregarding things for two months) and even after my husband going Tues and letting his office know how sick I am and after me telling them I can't do this for days, he called in zofran and he knows it hasn't worked in over three years. He didn't ask to see me, nothing. In tears and don't know what to do.

Yeah, I think you should go back to ER if you can't hold even fluids down. A PCP is a general doctor, like a GP, right? GPs are largely clueless and certainly not equipped for dealing with complex or rare medical cases. Hopefully the hospital can get the ball rolling, but don't let them send you home with the equivalent of paracetamol or zofran. Best of luck, I hope the nausea and vomiting goes away soon.

lizzie- surely sounds to me like you need to go to the ER where your GI doc and colorectal surgeon work out of, and you need to demand to get admitted.
simple as that.
the longer you wait -the worse you can become, then the harder it is for you to get better.
so get to the ER NOW, demand a colorectal AND GI consult when you are in the ER room (or have your hubby demand that for you) and then get admitted.
do not bother with your PCP. They are useless for complicated cases. You need help now. Once you get admitted, it will be a lot easier for the specialist doctors to collaborate, order the tests you need, and to get a resolution.
Just make sure you go to the ER and the very best hospital you can get to.
start packing and get in the car now... please

Trying to find a way to ER. Normally my husband or mom take me, but my husband had to work, no choice, and we've been watching my nephews for my parents (they have custody if my older sisters boys) because my mom underwent surgery yesterday on her cervical spine. So I don't have anyone right now. My pcp (should have explained) is the doctor who took on my case a year ago because all he does is take on complicated cases, but he has been doing nothing for three months. My CR surgeons nurse said I cannot go on like this and need to be evaluated in ER :( I have no energy to take myself and afraid as I'm weak and dizzy. Working on it, but this is bad timing. Thank you so much for your responses. I tend to wait too long...nurse in me...tends to think things will just get better. I will keep you posted. Trying to think of a way, but idk who to ask?!

Ambulance or taxi?

I know about waiting until things get better, but I would go with your instincts. This is not something that is going to go away on its own. A&E (UK equivalent of ER) is absolutely full of timewasters, people sitting around merrily chatting and laughing with their friends or family. Whatever trivial complaint they had no doubt could easily have waited until they got to see a GP. You are not one of those timewasters. I would not begrudge you being in front of me. So call one of the above and get admitted!

I'd say take the kids with you in a cab, ambulance if you need it. Someone else can get them later.

As a nurse, you understand that it's often hard to think clearly when you're very sick, and that's also why you need to be in a hospital. You just need to get yourself there. If it's a decent hospital, there will be social workers who can help sort things out w/the kids until someone can get them. Doesn't sound good for you to be worrying about taking care of children now.

This night has been a disaster!! My husband came home with my nephew and they had gotten ice cream. My husband started having stomach pains and tried laying down. He thought it was gas pains, but after an hour he puked everywhere from the kitchen to the couch, on counters, cabinets, drawers, and soooo much! Thankfully we have wood floors, but it was everywhere. My husband couldn't even move so I had to clean up everything that took about an hour. Gave my husband phenergan and he laid down and hasn't moved and is still in pain. I think he has the flu! and now on top of everything, I'm exposed to this nasty flu that in Nov landed me in ICU stepdown. I had to call my dad to come get my nephew as I couldn't take care of him alone. He needed a bath and dinner and I had nobody to help. My dad just came to get him and had to get back to my mom. I'm in worse shape and going to ER in morning as I know I need to most likely be admitted. I tried to drink 5oz water, but curled in a ball trying not to puke. This has been a nightmare. Thank you guys so much for your caring words. I'm too embarrassed for no good reason to call an ambulance and I'm a VERY long taxi ride away. My friend is bringing me in the morning she said. I will keep you posted. Love to you all.

Dear Lizzie,

I am very relieved you are going back to the hospital. You have already been through so much! I am a little confused about what intestinal tests were done on you. Your symptoms are exactly what I had with small intestine bacterial overgrowth (SIBO): nausea, vomiting, bloating, painful gas, chronic diarrhea, and horrible smelling excrement. The pain was outrageous too. I had malabsorption of vitamins B12, D and iron. I kept having UTIs also.

I tested negative for C-Diff and our hospital did not have the machine to test for SIBO so my doctor just went ahead and treated it with antibiotics. I did not respond to treatments with either Cipro or Flagyl. The good news is that symptoms improved within 3 days the first time I tried 1200 mg Xifaxin daily, the second time improvement was much faster. A pouchoscopy showed negative results for pouchitis but that is what is written on my chart. The treatment is the same.

The medicine is very expensive. My GI always gives me enough samples to complete the treatment. Otherwise I could not afford it. The manufacturer has a financial support program for patients with chronic infections. Your doctor should have their 800 number.

I also take VSL#3 Double strength twice a day. My insurance covers it. If it didn't, I would take Culterelle or Floriculture. I think both are available at Walgreens and they have the same bacterial strains. The probiotics encourage regrowth and balance of the "good bacteria" in your guts. The biggest improvement seemed to be from increasing my Vitamin D supplement to 6000 IU a day. I use a daily liquid vitamin B12 and iron. I am tested once a month. My results are still lower than normal but I have stopped having any UTIs.

Until you have stopped throwing up for at least 24 hours you may want to stay away from solid foods. My experience was that every time
i tried to eat something simple but solid I started throwing up again. We made homemade Gatorade popsicles, ice chips, and regular cheap not juice popsicles. Suck on them very slowly. Zofran does not work for me. Have you tried 25 mg Promethegan suppository? Even after the throwing up stops stick to clear liquids for a day: broth, jello, popsicles.

Next I graduate to eggs, toast, apple sauce, rice, and small amounts of banana.

The best pain relief was Norco. It is similar to Vicodan but without the
Tylenol. It is recommended for anyone with increased liver enzymes. I stop needing it as soon as the antibiotic starts working.

If you need peer reviewed published research to show your doc, just let me know. All but one of the five doctors I had in the hospital basically threw up their hands and said, "I give up." SIBO is real and it is treatable. The connection between extremely low vitamin D levels and infections is being strengthened every day as more research is being published in reputable gastroenterology journals. This is something very few GIs are aware of unless they teach at a medical university.

Thank You!!! I was up all night with my husband and then slept two hrs and woke up at 8:30am dry heaving every 15 min. I was so weak and dizzy. I could not get ahold of my friend and was going to call 911, but drove myself. It was stupid...I almost crashed twice, but got right back and labs show severe dehydration and vitals were not good. Resting HR was 135-140 and high BP. I was down 7 lbs from Monday and 20+ in month. They bolused me with 2liters, IV dilaudid, and phenergan. Did CT with IV contrast and that seemed ok, but I've had ZERO output since 7:30pm last night and no urine output. They decided they don't know what's wrong, but transferred me via ambulance and admitted me. I have no idea what's wrong and I've been in tears and alone and this sucks. Thank you for the info Cynthia...I will def tell them. I asked about it to my colorectal, but they seemed to not think so? I have every single one of your symptoms. I've had low vit D for quite some time. I do take PO phenergan at home, but even that wasn't working. I will keep you updated.

Sorry to read this. Hang in. I am asking my local doc about xifaxin this week. Makes sense if sibo bacteria is backing up in us then it could explain a lot. So far am still able to sqeek thru liquids but i have to stop the noramal pulp free oj and get the reduced sugar pulp free oj. I had a tiny bit of a kid size soft serve and look 9 months pregnant. The sibo bacteria go wild on sugar.

I hope they listen to you. Cynthia great post and very informative.

lizzie- though I know it really stinks for you but I am very glad that you have been admitted to the hospital. Next time I'm going to slap you for getting in that car when you are so poorly. I did that once myself, in similar state as you were and it was probably the dumbest thing I have ever done. It was my trip that landed me from ER to admission to colon removal surgery 5days later for the worst massive UC flare I ever had.

Anyways, I doubt that you simply have SIBO. I think you have more stuff possibly going on. Also usually colorectal surgeons are not sibo educated, that's more of a GI docs area. I have been recently diagnosed with sibo. Suffered about 6 weeks before getting help. Serious massive belching - worse than any beer drinking male you have ever met, horrid abdominal distension - looking like octomom before she had the 8 kids, terrible pain, absolutely zero appetite, if I ate one bite it felt like I had ate thanksgiving dinner, horrid gas in the ileostomy nonstop, joint pains, and significant weight loss. Anyways GI doc did another ileoscope to rule out crohns again. He never bothered with sibo breath test. He just suspected it and then prescribed xifaxan. It helped within about 3 days. Problem is, after my two week course, all the symptoms came back within about 2 days. Started taking it again - for about 20 days. and have been off it now for about 5 days, knock on wood, no symptoms returned yet.

My Cleveland Clinic GI doc said many people have to be on antibiotics for their life with SIBO, but he recommends pulsing antibiotic doses. Say taking 2 weeks and getting off them and then back on antibiotics. Also you must cut out ALL sugar. And I mean ALL sugar. Zero sugar. even something like "reduced sugar pulp free oj" would be a huge huge mistake. And stick with low fiber and very high protein diet. Tons of meat.

Also Xifaxan is covered by my insurance. lucky me. So no problems with that. Unlike Cynthia I do not have low Vit D levels, nor low B12. As I have been taking vit D supplements for several years and do weekly B12 injections since I had my jpouch ripped out. My GI doc suspects its from massive adhesions causing poor motility through the small intestine resulting in bacteria buildup. I have had many many abdominal/intestinal surgeries. I still don't understand why sibo struck when it did, but guess it doesn't really matter.

lizzie also when I was in the hospital a few months ago for abscesses and on potent iv antibiotics and couldn't stop vomiting for days and zofran didn't help, and phenergram didn't help.. one of the nurses finally suggested reglan. Guess its an old school drug. But it helped me. Maybe ask for that if you are still vomiting. Its worth a shot.

also wondering is there an infectious disease doctor on your case. Just an idea.

This is lizzie's'thread and i am not meaning to highjack it but i just wanted to thank you for listed ALL your sibo symptoms. Explains ALL mine and why all is worse by night. The pain, no appetite, belching, ditension. I will call dr. Shen's for the xifaxan rx to try. What dosage did you take?

I have to live on BoostVHC which has the least amount of sugar and the most cals and protein that i can get, but i will cut out all those juices as this sibo stuff is brutal. I HAVE to take the chance on this stoma redo at CC tho Dr. R outlined all these horrid things that could happen. I have to have this stoma opened up or i will starve, burst or die of this sibo pain. Makes sense the backed up waste behind the stenosed stoma with the 2 kinks that dr shen found. Explains a lot. Does the xifaxan come in a dissolvable tablet?

It sounds like we were admitted to the hospital at the same time! I wish we were in the same city so I could give you more direct support. I lost the vision in my left eye from a bad migraine and could not find my migraine meds. (We just moved two days ago.) The ER people were so uneducated about j-pouches. Both the doctor and nurse asked me to lift my shirt so they could see the infection! Luckily my colorectal surgeon checked in and told them what I needed.

It might be time to try Reglan. Long term use can cause permanent nerve damage but short term use has few side effects. My youngest son has gastro paresis. He can vomit for up to seven days. The doctors often alternate Reglan with phenergan for him. It sounds like tpn might be a great idea for you too. Your guts need to rest and you definitely need some calories. You may end up with a NG tube to help clear out the acids in your stomach. I never had the NG tube for more than 8 hours. Having it put in was a little uncomfortable but not at all painful. Ask for someone with lots of ng tube experience.

At the point you are at I was usually on morphine, saline, phenergan, lorazepam (for anxiety and to relax all of the muscles around your digestive tract), antibiotics and an antidepressant. I do not understand why but I would have crying jags that would go on for hours. I didn't really feel depressed just exhausted and overwhelmed.

My back was often very sore from all the throwing up. The nurses would pack hot blankets around the sore muscles. The heating pads available at hospitals are terrible, they are not warm enough to help. The hot blankets were heavenly.

Please give us an update on how you are feeling. I am still on morphine so I hope this post makes sense!

Sometimes having lots of doctors admit to being stumped means they come up with creative and effective solutions. I rather have a doctor who is honest than one who pretends to know everything.

I got out of hospital late Thursday night and things are no different. It's a very long story. Somehow with five days of fluid I ended up losing more weight. I was getting great care until they called my PCP and he had them take away all my Meds: pain, nausea, etc. I am extremely allergic to Reglan :( The palliative care team had me on dilaudid every two hrs, iv Ativan for nausea, iv phenergan, etc and I was very comfortable until they called my PCP, whom I said isn't your typical PCP. He took on my case a year ago as he loves complex cases, but he told them that its all psychosomatic. He emailed my husband back (husband emailed him very angry) and said awful things. GI was consulted and did upper scope that was negative besides a clip left in my stomach?! Most likely from my feeding tube. My vitals were stable for first time in months, but after they stopped all Meds my HR has been back between 110-125 resting and I'm very dizzy. I prayed for someone or something to show that I'm not making these symptoms up. My PCP said my labs/weight don't reflect someone with nausea/vomiting syndrome...interesting because he hasn't seen me in month!!! Since then I'm down ~22-24 lbs in six wks. GI recommended some manometry test, but not done in our city. After crying myself to sleep...the next am a dr as cute as can be with a bow tie introduced himself and said he was from endocrine. I was told they cancelled the consult. He saw me for two hrs and explained my PCP and former endo doc I had recently seen did my labs incorrectly and I do in fact have disease from tumors on adrenal gland. He was appalled at how they've treated me and said he had to admit he doesn't do hospital consults, but after looking at my levels, said he knew I was a pt he could put in the charts and one talked about at national conferences. They drew seven tubes of blood, I have to get picc line and CT of adrenal tumors today at 3pm and see him May 22nd to do further testing and will most likely need surgery. I saw urology yesterday and he believes my entire bladder is covered in adhesions causing me severe pain. As far as GI?! I don't know what to do...I still can't eat/drink and just continue to lose. I've never had such retractable nausea. I see my CR surgeon May 15 and hope that he can refer me to GI. I'm so discouraged, sad, angry, hurt, and frustrated I have nobody to help me with nausea/vomiting b/c my PCP told them I'm just overstating. I have nothing to gain from lying. I'm so weak and my mom is scared I'm going to collapse or worse, but she can be dramatic. Anyways...I need advice how to handle this?!? My normal hypertension is super low BP now from no intake, but heart racing. Thank you for your support.

I'm soooo frustrated :( Everyone in my family just keeps telling me you HAVE TO EAT! And drink. I'm trying so hard, but the pain and nausea is enough to make me not want to eat period. I have no idea what happened. In Feb I was actually eating better than I had in years and nausea was under control with phenergan prn. I wish I wasn't allergic to Reglan, but I had horrible side effects immediately after first dose. I had same reaction to Bentyl. I know domperidone is another med in Canada used like Reglan, but unsure if it has same side effects or how one gets this medication. I had my picc line placed and CT if Adrenal glands. My CT of abdomen and pelvis did not show obstruction. It showed thick walls of bladder and vaginal walls with congestion. I know this is all repetitive, but my family just wants me to force food and I don't have anyone to really talk about this with. My dizziness is somewhat better this evening and I was able to drive to CT, but even with a few candies in my stomach I'm so nauseated and May 15 seems so far away to see my colorectal. I hope he will refer me back to GI and maybe they will have some suggestions. I am only emptying my ileo bag once a day and somedays every 36 hrs, but I'm sure that's attributed to lack of intake. I'm just afraid I will end up back in ER in a few days if I can't get on top of this? Whatever may be going on :( Sorry to vent.
