Hello, I've been using a one-piece system for most of my time with my ileostomy. I want to order some two-piece pouches just to have on hand and to experiment with. My experience with two-piece systems are only whatever the companies sent me as samples, and my chief complaint is that the plastic ring is uncomfortable and inflexible when I bend over. I see that there are different sizes of the coupling (the ring). I'm looking at the Coloplast Mio Click, and it has four different sizes - 40, 50, 60, and 70mm. Why are there different sizes at all? What size should I get? Would a smaller size make the ring less noticeable and more comfortable? What is the downside to a small size? My stoma is 1 1/8", if that plays into things. I need some two-piece expertise here. Thanks!

Basically they are made different sizes as people have different size stomas. The size you should order is size next up from the width of your stoma. So if your stoma measures 45mm across as it widest then you order the 50mm ring size. If you get the one that is the right size you won't notice it as much. I use the convatec flexible flange two piece system. Instead of a hard ring,it has a softer one and inside that is a flexible collar that fits snugly over the stoma like a polo neck jumper so it keeps a good fit as the stoma changes size and shape throughout the day and night, saves having to use paste or suchlike.

Okay, thanks. I think that's what I was missing - that you should get the next size up from your stoma; or basically the smallest size possible where it still fits. So, with my stoma being 1 1/8", the Coloplast Mio 40mm cut-to-fit does stoma size up to 1 3/8", so that is what I will try. It looks like the two-piece wafers I tried had a 60mm flange, which is very big. Maybe I'll have more luck with the smaller flange.
