I understand your concern, his as well, but the changes in size and shape -are- normal. And no, the stoma cannot fall back into the body without some serious force applied first. My previous stoma was practically flush to my skin and I was concerned about complications while I was being looked over by the doctor (she wasn't gentle), and she assured me she'd have had to really pushed to pop it back into my body.
The stoma is sewn to the abdominal wall, or opening. Your boyfriend might have noticed stitches around it. But if he's worried about skin irritation, a convex wafer and belt can be employed to kind of 'push' the stoma out a bit more. The burning pain is probably from it being so close to his skin. Does he notice sores when he changes his appliance?
