I'm very sorry to hear that you are going through this. I too have an ileostomy but still have my rectum. I have hear of this phenomenon many times it is possible that you have phantom rectum syndrome. You might have heard of phantom limb syndrome it is similar to this. Basically you either have damaged nerve endings or as a result of activity within the brain as it struggled to deal with the fact that you have no rectum. It's good you're seeing a pain specialist, they could suggest gabapentin or something in that family for neurological pain. Just remember that it is very hard to treat a phantom organ. again really hope you feel better soon.
