Possible Blockage Again?

So for the third time, I think I was hit with a partial blockage. The odd things is, the symptoms and the way they hit me are identical with all three instances and I'm not sure if something else is going on. about a month ago I got hit with these symptoms but they subsided before I could go to the hospital as my surgeon wanted. Last Wednesday, I ate lunch about 11:30 am, I was out of town working. At 8pm I was still "tasting" lunch so I skipped dinner. I woke up about midnight with the all too familiar abdominal pain and nausea. The pain feels like someone is standing on my stomach with all their weight. The pain and nausea feel the exact same as the previous two incidents. The pain lasted till about 6am after keeping me up all night in extreme discomfort. For most of the day Thursday I felt very run down but no pain except for a little abdominal soreness but as the day went on, I started to feel like I was getting sick. I got home late Thursday night feeling completely run down and checked my temp, it was 102. That fever lasted all night alternating sweats and chills and finally breaking early Friday morning. Today I just feel really weak and tired. These symptoms have been exactly the same on all 3 incidents: pain/nausea > pain/nausea goes away > feel sick and develop a high fever > fever breaks > back to normal in a couple days. If I wasn't out of town I would of gone to my surgeons hospital but it wasn't an option. I have an appointment next Friday with him to pick his brain. Does this sound odd to anyone or sound like a typical blockage? Some say they develop a low grade fever with blockages but not as high as mine have been

Checking in...seeing how you are.

It sounds like a partial blockage except I have never had a fever with mine.

Please let us know how it goes.

Hope you figure it out and don't have another episode.

I'm scheduled to see my Surgeon on Friday and my GI next week. Some on another site suggested it could possibly some sort of gallbladder issue. The symptoms do fit to a point so I'll bring that up with my doctors. It's just so odd that the symptoms are completely identical and in the same order process every time. With a fever that high the body is clearly fighting something

I am concerned about you. I wonder what's going on??? How are you doing now? It does sound like it could be a partial blockage, or it could be some kind of pathogen. I hope you can get to the bottom of it. I actually ate a bad taco the other day and I woke up in the night with horrible sweats and chills. It passed really quickly at least. It could be many things. I hope you're feeling better now at least.

as soon as day 4 rolls around, I feel perfectly fine again. I thought about a bug but I would expect vomiting and diarrhea to which there was none. I only vomited once last September when I went to the hospital for the same thing. The cramping is brutal and does not go away at all, it's constant the whole time. I'm just reading now about Pancreatitis, which includes: abdominal pain, nausea, vomiting, fever and chills.

saw my surgeon today, he drew some blood and is sending me for a CT scan next week as well as a HIDA test. Said with the fever that high it's not a food blockage and definitely sounds like a gallbladder issue

I got my CT and HIDA scan and it showed my gallbladder not functioning properly. It was "abnormal". There were no gallstones. So, my surgeon wants to discuss removing it etc. I'll see him in a couple weeks after a trip I have coming up. Once I understand exactly what's happening and if there are no other alternatives to correct the issue I'll see about having it removed by him.

That's crazy G, but probably better than having a problem with your pouch. It sounds like it's a serious procedure too though, I am sorry you're going to have to have another procedure.

Saw my surgeon yesterday. My gallbladder isn't contracting and releasing bile normally but he said in theory, when they gave me the drug during the x-ray to contract it, the symptoms of the attack should of happened but didn't. He's sending me to my GI for an endoscopy more than likely. He's still stumped on the high fever that follows the attacks. Removal has been postponed. He said removing is more risky with me as things have been "moved around" due to previous surgeries, so it's wait and see till my next appt.

Were your surgeries open Surgeries? probably a lot more scar tissue that they to work with and around.

Yes, my pouch surgery was open due to scar tissue that developed from my bowel resection back in 2013. It took him an hour to get through it all. Not to mention my surgery site was opened a second time to find the leak that was happening.

The only thing that I can find that relates to my symptoms the best is pancreatitis which I believe go hand in hand with the gallbladder. That has symptoms of the same abdominal pain, nausea and also includes a fever of 101+. It also says it can mess with your blood pressure which may explain after the pain goes away I feel very light headed and dizzy

Any progress on a diagnosis? Have you had any additional "attacks"?

I can't get in to my GI till the 11th of June so have to wait till then to see what direction or insight he might have. So far no more attacks, just the 3. It's so odd that they have been so far apart with no real common factor that could be a trigger.

I haven't seen my GI yet but was looking over the results from the HIDA scan. "A homogenous distribution of radiotracer was noted throughout the liver. No focal photopenic defects were noted. Activity was noted within the gallbladder at 35 minutes post injection. Activity as noted in the bowel at 15 minutes post injection. Normal drainage of activity was noted from the liver. Subsequent sequential imaging was performed following IV administration of 1.6mcg of CCK. The gallbladder ejection fraction was 3% which was well below the normal range consistent with biliary dyskinesia."

Saw my GI yesterday and he also brought in another GI to go over my "attacks". He does feel it's a bile duct blockage of some sort. He was asking if I had liver enzymes checked at all which I haven't. He also asked if my urine had changed color during the attacks but I never really paid any attention. He feels there's another issue beyond my gallbladder ejection at 3% being the cause of the attacks as they are so far apart. The fever, he explained that he thinks bacteria is being pushed back into my bloodstream during the attacks and that's what my body is trying to fight. I did have salmonella issues many years back which was brought up. If you're a carrier for salmonella, the cells live in your gallbladder. At this point, there is no plan for removal. He gave me a prescription for a few tests that if I have another attack, I'm to bring that script to the ER for those specific tests to be run while I'm symptomatic and go from there.

If it's something to do with bacteria, would an antibiotic help to stop the attack? I wonder if there's at least a solution for treating it. hopefully the tests give you some more clues. I guess it's good that you don't have to worry about another surgery at the moment. it's nice that your GI was able to bring in another GI to improve your chances of figuring this out.

I'm not sure about the antibiotics. I believe the bacteria is a byproduct of the blockage if that's what's happening. I think if the bile duct is being blocked which causes the pain and nausea, that blockage generates the bad bacteria and sends it to my bloodstream. That would make sense I think because the fever doesn't kick in for a few hours after the pain/nausea goes away. By that time the bacteria is probably taken hold in my blood and my body begins to fight it. I think he just wants to get a more definite answer as to what's exactly happening before saying go ahead with the removal considering all I've been through. Honestly, I wish I could get it to trigger so I can get the tests done for definite answers. My big fear is this happens when I'm traveling for work and can't get to a hospital for the tests.
