Sometimes the stoma just bleeds and can look more than it actually is. The stoma does swell in size as bowel moves and when bag is on you don't normally notice this. Ensure that wafer hole is cut to the right size and that it is fitting properly and not rubbing the stoma. Many people go a week with same wafer, it is fine to leave it that long as it better for skin as changing less means skin damaged by taking wafer off.
As long as it is still keeping output as normal level and no pain then just keep an eye on it and ensure it is back to normal ion next change. If there is free blood in bag then I would ask doctor/nurse just to put yyour mind at rest.
