Tear in my ostomy...HELP

I have had an illeostomy for two years, however; in the last two weeks I've noticed a tear in it. It's about an inch long and bleeds and has mucus coming out of it when I change my bag. It also makes me naseuas when I change my bag. Any suggestions??? Thank you.
