Have you met w/a good ostomy nurse? That's the best way to get individual help.
I would think witch hazel would be harsh -- do you dilute it? I used to use that on my face some, and it says to dilute. You can wash instead w/just water, or water w/a mild soap like ivory or dial. When I don't shower, I use the safe and simple peristomal wipes, which leave no residue and are very gentle (insurance covers them).
Are you measuring her stoma every time? The wafer should be cut about 1/8 of an inch larger than her stoma, but her stoma will shrink as it heals and the swelling goes down.
Have you tried a different brand of pouch? Different brands work for different people.
Are you using a lot of the no sting barrier? That can affect the adhesion of the wafer if there's too much of it. I was having leaks at first and I cut down on the number of products I was using and that fixed it.
Many people find heat helpful after applying the wafer to help it stick better -- either using a hair dryer or heating pad (not too hot).
Is her belly soft? Is her stoma short? In either of these cases, she might need a convex wafer either to help push out her stoma or to make a better surface to adhere to.

Hang in .... it's very frustrating, but in almost all cases, fixable.
