Are you able to pass output through the drain tube?
Even if you decide to go on TPN the intestines will continue to produce waste (sloughing of cells, mucus, digestive enzymes, etc) that will need to exit as output.
I had to undergo several surgeries (unexpected/emergency) in succession after an extensive intestinal peroration with peritonitis. Subsequently, I developed abdominal adhesions. I endured several partial/full small bowel obstructions in the first several months after surgery. Very frustrating. However, the passage of time has eased the number of obstructive episodes that I experience and the episodes that I do have are less intense. I guess what I am suggesting is that time and patience (if you have the option) without additional surgical trauma may find you in a better position a few months from now then you are today.
Whatever decisions you make, I hope for you a brighter and better horizon.
