I had very watery output with my loop ileostomy but also blocked often. I was up to 8 Imodium per day at one point but had to back off down to 6 per day because of the blockages, and even on 6 they cropped up a few times per week. The loop ileostomy can place the intestine under a lot of artificial tension that significantly increases the likelihood of blockage. For what it's worth, this is less likely to be caused by an actual mechanical or inflammatory stricture (narrowing), moreso by kinks in the configuration of the intestines. My surgeon was confident that once my loop was reversed and my intestines were no longer under tension the blockages would stop occurring and they did.

When is the pouchogram? For loops that are high output or hard to manage many surgeons will move up step 3.

Most likely the same way that small bowel strictures are diagnosed in CD, which is typically CT. That said, why would stricturing or inflammation in the small bowel start all of a sudden now, when it's well known that blockages can be caused by a loop ileostomy under tension? I never had any kind of diagnostic imaging when I was dealing with it because my surgeon felt it was so obviously related to the loop.

Dehydration tends to beget nausea which can worsen the dehydration. I certainly wouldn't rely upon the ER to diagnose anything GI (or to even know what an ileostomy is half the time) but IV hydration sounds like it could help her right now. Some urgent cares or PCP offices will do IV hydration too.
