People with jpouches have their rectum removed and a pouch is created for stool collection. The pouch is made from the end part of the small bowel. The only piece remaining is the cuff, which is usually a couple centimeters. That can get inflamed, or the pouch itself, but that would be considered cuffitis or pouchitis. I think you need to go to your surgeon and have them take a look to see what you're dealing with. Inflammation in the pouch, whether it was the cuff or the pouch itself, wouldn't mean you need to have surgery. There are treatments, often curative. Some people have it chronically, even in that case, there are medications you can try.
Are you on any drugs that would cause the hunger? Have you had blood work to check for other issues that could cause weight issues, such as thyroid?
I've had the opposite since my jpouch surgery. I seem to have a lower appetite. I had really bad side effects from prednisone and my weight was up and down like crazy, I was always hungry. Since surgery, I've completely leveled out. I am back to my "old" (preUC, PrePred) eating habits. With a jpouch, you go to the bathroom more, I would think it's a little harder to keep weight on.
