Question for those whose UC was most active near rectum

I'm on year....oh 12 (I've lost count) with UC. I recently left a drug trial (jak inhibitor) b/c I got worse. My UC has always been limited to 25-30cm. With the most severe inflammation 10 and below. I'm not quite mentally ready for surgery but if I do will that "cure" the severe UC I have in the rectum? Would I even be a J-pouch candidate with my disease so active there?

Hopefully this makes sense. Trying to get my ducks in a row before my next appt.

when you get a J Pouch, you will lose your entire colon including your rectum except surgeons like to leave a small rectal cuff which is about 3cm. It is possible for UC to still attack there. The cuff is left to help with bowel movement control

UC always starts in the rectum, everyone with UC & a J-pouch has rectum involvement. It shouldn't be much of an issue unless your rectum is damaged.

If the rectal cuff is too big then you could end up with some flaring (Cuffitis) which is usually treatable with suppositories. This is one of the reasons that you need to find a very experienced J-pouch surgeon. I've heard of some surgeons leaving 10+cm rectal cuffs and people having a nightmare of a time and needing to get their pouch revised (2 additional surgeries).

My UC was limited to my rectum, about the size of a quarter, and that small patch led to surgery. I rode out a 2+ year miserable flare, I never imagined an ulcer that size would lead to surgery but it did. That was 16 years ago. I have a jpouch and life has been great post surgery; I have never regretted my decision.
