Dealing with issues; Cancer and prostatitis

I am 66 yrs old with prostate cancer and prostatitis. Regarding the cancer, I have come to terms with it and comfortable with my plan:

My Gleason is 3+3=6. After getting the "news", talking to others, researching options and consultations, I plan to have Davinci robotic surgery performed by Dr. Menon at the Vattituki Institute of Henry Ford Hosp in Detroit. Before the operation I need to be completely free of the infection and to loose more weight. I lost 65# since January, 2011, but a little more will reduce risks/breathing, etc. Since killing the infection and loosing weight can't happen overnight, and my Gleason score did not call for urgency, Dr. felt waiting till March/April would not be a problem. (longer term "watchful waiting" was not advised)

Following years of PSA in 3 range, it rose to 6.0 last year. Digital exams by family doc over 10 yrs yielded normally enlarged (age related) prostate. Although I can't write my name in the snow or hold it all day like an 18 yrs old, there was no getting up at night or start-stop issues. Family doc sent me to a urologist who did a more thorough exam (uncomfortable) yielding greater enlargement on one side and recommended biopsy.

I chose a different urologist because I wanted a provider with color doplar ultrasound. This was advised by a friend who had it done. This urologist had me take Cirpo for 2 weeks and repeat PSA test to rule out infection causing high PSA. It was the same and biopsy was scheduled. I continued to take Cipro through the biopsy and 4 days after along with another antibiotic (can't remember name)

I used a Fleet enema prior to the biopsy. The biopsy was uncomfortable but I survived. Before leaving office, I was able to urinate. Blood in urine and feces continued for several weeks. I didn't discover blood in ejaculate for several more weeks as I wasn't in the mood. This eventually did happen and was bloody but went away.

After blood in urine subsided I noticed increased frequently and urgently with discomfort (slight burning). I developed a fever (101.5) and felt lousy. Went back on Cipro again. Symptoms (fever, etc) decreased in a couple days and I continued Cipro for 15 days. A week after stopping Cipro, symptoms returned. Septra was prescribed to be taken over 3 months, ending Nov 20.

By Dec 1, frequency and some burning returned. I called the urologist wanted me to go back on Septra until the operation in March. During my research and consultation with several doctors including Dr. Menon, he asked if my urologist had ordered urine tests prior to starting antibiotics. This wasn't done. Because of this, I did not start the Septra immediately. I felt uncomfortable "second guessing" the doctor and instead called Vattikuti where I spoke with Folushio (one of Menon's team). He faxed an order to a lab local to me. Once I peed in the cup, I was to start the Septra. Even if it wasn't the right one for the infection, it did reduce symptoms which by then had increased and temp was up. I'm still awaiting the results of the urine test and whether I need to be on a different antibiotic.

Taking the Septra has reduced my fever, etc to normal and the frequency somewhat. However, I do have an urge to move bowels more frequently. I "feel" as if something is there. I am drinking lots of water with the idea of trying to flush infection out. However, I'm not sure water will clear seminal fluid in the prostate and "flushing" that is a different process.

After my temperature came down, I did attempt to "flush" my prostate. Since there was discomfort in my pelvic area, my partner offered to satisfy me orally. This proved easier because I could also not achieve full erection for penetration. I have always been able before, except occasions where we tried a second (or third) without enough time to revive. In these 2nd or 3rd situations, although not completely firm, oral stimulation always brought me to climax.

This time, as I built towards an orgasm, there was pain in my prostate; so much so that I asked her to stop. I lost my erection completely and it hurt slightly for several hours. That was a couple days ago and I'm not sure when there will be another opportunity. (It's complicated).

I read an article about "why doctors don't want to treat prostatitis" and am concerned. Prior to the biopsy, my only symptoms were normal loss of pressure and going more often. I'm almost wishing I never had the biopsy. As evident by detail I included, I'm not ready to be a monk. I have accepted the cancer and chosen the best alternative for me. I also understand regaining capability after surgery will take effort. My partner is considerably younger and willing to help me with this effort even though our situations preclude daily or even regular contact. (as mentioned, it's complicated)

Right now prostatitis is my primary concern but that doesn't reduce the number of worms in this can.

Any suggestions / advice on any issue I raised?

I wish I could better understand the infection. Nothing I've experienced would send me to the hospital and the antibiotics have taken care of any (fever/headache, lousing feeling) each time. Since the bots work, why does the infection come back?

In regard to sex being a larglely mental, it wasn't my mind that hurt and caused stopped my orgasm. It was my prostate.
