Hi All,

My husband is starting to have an increase in his Psa levels , And a second line if therapy will probably be needed,.15 months after surgery he is at 0.14.

Can anyone give me some Medical and radiation oncologist recommendations in the NY or Westchester County Area.

 NYC is full of competant Drs., but finding a Dr through someones elses positive experiance is always helpful. That is how we found our surgeon, and he was and is an amazing man and surgeon.

Sorry that  I can't help with a recommendation. I am in norhtern CT. When faced with a rising PSA two years ago, I decided to go to Dana Farber for SRT and saw a medical oncologist there, prior to and after SRT. I also decided to see Dr Myer in Charlottesville for regular consults.

Hi BBFan,

Dana FArber is in Boston , correct ? Did you make that trip every day for SRT.
I'm new to the SRT and HT so I really don't know what's involved.
Do they start HT first than IMRT ?

Had RP in 2011 with rising PSA in 2012

How was it going through that therapy ?
I know that is where I am headed.

Dana Farber is in Boston. I considered Dana Farber, MSK and Yale Cancer Center. I think that if I was closer to NY I would have gone to MSK. I also considered my local facility at Hartford Hospital, but decided that since I may be in this for the long haul, I wanted to move my treatment to a top notch facility. I did HT (ADT3) with SRT. HT for 1 year starting a month or so before SRT. SRT for me was a piece of cake. Folks at Dana Farber (actually SRT treatments were done at Brigham and Womens Hospital around the corner) were very careful with treatments and made sure that I prepped adequately. HT came with more side effects, but they can be managed. I can tell you that I was happy to be off after 12 months. The good news is that I have mostlty recovered from the effects of treatment and PSA is undetectable. I took the train most days from New Londen to Boston and walked to the hospital. It pretty much shot the whole day and I didnt work during SRT. My employer was good enough to put meon short term disability. I did have a consult with DR Myer and continue to see him. I did my rt at Cornell ( dr Parashar) Was very happy with him. For oncology post RT I am going to Columbia (dr. Petrylak). He Is excellent

Thank You Keeping faith for the recommendations. I will look  up both those oncologists.We also have Mem Sloan Kettering.

Dr. Michael Morris at Memorial Sloan-Kettering.  He is a great doctor as well as a scholar of the disease and a real gentleman.  You can read his write-up on the Sloan web page.  Phone number for his office is 1-646-422-4469.  His offices are at 353 East 68th St, just off First Avenue.  I live upstate (near Albany), but I am all in with Dr. Morris and Sloan-Kettering.  Dr. Morris has a special interest in men who have had surgery, and subsequently saw a rising PSA.  I cannot recommend Dr. Morris or the Sloan team more highly (just finished a course of radiation).  Best wishes!

DaVinci surgery 11/09. Clean margins, clean seminal vessels.

Rising PSA noted 06/11

Gleason 8

Time to recurrrence 18 months

Three month doubling time (summer 2011).

PSA rose from .07 on 06/11 to .17 on 11/11.

MRIs and bone scans negative so far.

Started Lupron/Casodex 12/11

Began IMRT 02/20/12.

Finished IMRT 04/16/12

Follow up in July, 2012.

Further to clarify.  Dr. Morris is a medical oncologist.  My radiation oncologist at Sloan Kettering is Dr. Marisa Kollmeier -- 1-212-639-3952. I just finished a two month course of radiation + six month hormone treatment -- went great.  Best wishes again.

Did you have your surgery ther also ?

I am a bit confused though. Are the radiation onc. the ones that do the radiation, or do they just prescribe the course of radiation.Since you have to have about 40 treatments you can't be traveling all that way each time.

Sorry if it sounds like a stupid question,but don't know much about the SRT yet just starting to educate myself. (And I thought picking the surgeon was hard !)

How were your side effects ?

How are you feeling post treatment ?

Dana Farber would have set me up in a dormatory that they maintain for patients if I couldnt travel there daily. Im sure that there would have been some charge. They would not plan and monitor the radiation treatments unless it bwas conducted that their facility. Perhaps MSK operates differently.

Dr Morris was my original point of contact -- three meetings.  He referred me to Dr Kollmeir within MSK for radiation.  She set up the radiation scheme;  the actual radiation sessions are handled by a team of technical people (I liked them; we got on well).  Once a week I met with Dr. Kollmeir or her Nurse to discuss and review.  As I didn't have any problems, there was not a lot to discuss, but I did appreciate the weekly review.  My only side effects were slightly more frequent and urgent bowel movements.  Not a big deal at all. No accidents. No urinary problems.  Felt strong throughout.  Walked at least two hours a day in the city.  Some days a lot more. It will be three months at least before we know if radiation worked.  May be some time after that in my case because I also underwent hormone therapy.

Re lodging, we rented a short term apartment in the city on East 82nd St (Yorkville if you know the City).  Sloan lists several booking agencies for this on the Sloan website.  We used an agency named Furnished Quarters.  If you decide to go that route, I can give you more info.  Met other Sloan patients doing the same thing.  In treatment, I also met a man who was driving in each day from Westchester County. Worked well for us, staying in the City.

You had also asked about the site of the original surgery.  That was done in November '09 in Albany.  On the face of it, successul -- clean margins, clean seminal vessels, quick recovery, not a day of incontinence. Except, PSA emerged again after 18 months.  When it did, I thought my surgeon's response was inadequate and sluggish  -- he wanted to monit0r things for another six months.  It was at that point that I looked into alternatives at Memorial Sloan Kettering.

Thanks for your help. I think we will end up going the MSK route. We live about 40 mi North of NYC so probably the Daily commute. I still have 1 more PSA test before I will have to make any serious decisions. but unfortunately I'm not very optimistic.

Really thought that the RP would have kept me out of this for a lot longer. I will let you know, Hope you get good results. Praying for everyone I meet here.
