Biopsy result ?

First off, rejoice in the negative biopsy.

I am not a doc, just a semi-informed patient. I think there is something to be a little bit worrried about but it is not the atypical cells. Probably most 50 year olds have atypical cells. The risk is that there may be cancer but the biopsy didn't find it. That happens sometimes. It depends how many cores they took. If they took 6-8 cores, it's easier to miss something than if they took 12-16 cores. So I'd ask about that. If you can find how many cores were taken please post that info here. Also if you have information from prior year's PSA's it would be good to post that here. having a 3-5 year time trend can help sort things out.

There are several options at this point:
1.) continue to have PSA tests on an accelerated schedule. Like do it again in 3 months or six months and see if it has stabilized. PSA from a benign prostate should not continue to rise at such a rapid pace!
2.) it is possible that there was some other reason for the big increase. He might have a prostate infection. the doc might want to put him on antibiotics for a few weeks and retest. That would be reasonable. he might have been on a bicycle or had sex or done something else in the 24 hours before the psa test which would lead to a high result. Some docs are careless enough to do the digital rectal exam BEFORE takign the blood, which should never be done because it increases the psa artificially. All these issues can be checked out with a retest, carefully following the instructions.
3.) There are other tests like a "Free PSA" test that are inexpensive and easy blood tests that are typically done on people with an elevated PSA in the absence of clear evidence of the cause. While these tests are not definitive, they can give an indication, e.g. "probably benign" vs. "good chance the biopsy missed a cancer that is lurking in there." If the latter, they redo the biopsy taking more cores this time.
4.) They can do a calculation called "PSA density" which looks at how big the prostate is (using ultrasound) and compares that to how much PSA the prostate is producing. If the PSA is too high relative to the volume of the prostate it suggests there is some cancer lurking in there, which can then be followed by another biopsy, the Free PSA test, or another test.

So on balance you have several options for follow up. A good urologist will go through all this with your family member. If I were in his situation I would relax a bit but I would not relax completely. Stay vigilant and rule out the various reasons for a false positive. If it IS prostate cancer, and if it IS going from psa of 5 to 10 in a year, and if it continues to move at a fast clip like that, it needs to be found and treated in my opinion. Again, I am not a doc, just a semi informed patient.

I forgot to mention one thing in my prior post. Some docs believe that the drugs that are used for Benign Prostate Enlargement (like Avodart - look it up on wikipedia) will shrink benign prostates but won't shrink prostate cancer. So the strategy then is to put the patient on Avodart and see if the PSA drops significantly. If it does, it's presumed to be benign; if it doesn't it's presumed to be potentially cancerous and other steps are warranted to investigate further.

Lots of options...

If there is any concern with the capability of whoever looked at the biopsy slides, consider having them sent to another lab for a second opinion. Most health insurers will cover that and it may be worth doing. I will always wonder if I might have caught my cancer on the first or second biopsy had I gotten a second opinion.

Since you asked if anyone had similar biopsy results and went on to have PC, there are a number of such here and I am one of them. My first two biopsies showed only inflammation (no cancer) but my PSA kept rising, despite being on Avodart which usually makes it go down.

My third biopsy was positive in one of 14 cores for a small amount of low-risk cancer (Gleason 3+3=6). I had a second set of slides made from that biopsy and had it looked at by another pathologist and he found the same amount of cancer in the same core but he graded it GS7 (3+4) making it intermediate risk.

While I was trying to figure out what to do I had a fourth biopsy which was negative (but my PSA continued to rise).

I decided to have a DaVinci robotic prostatectomy and my post-operative pathology showed a large, high-risk tumor (GS9) with positive surgical margins.

So, my uncomfortable observation for your family member is that a negative biopsy doesn't prove anything. He still needs to aggressively monitor the situation. Statistically, a 12-core needle biopsy will catch 7 out of 8 cancers. His negative result gives him better odds but it doesn't mean he can cash in his chips and go home.

Was He put on antibiotics to rule out infection. Prostates can get hard to eliminate infections that drive the psa up. Psa of 5 is NOT normal and a doubling of that in a year is NOT normal. Ask for saturation biopsy that is at least sonar guided. This is NOT a lets take a looksie in six months situation. A dre, bicycle ride , sex all done the nite be fore at the same time will NOT cause a psa rise of this magnitude. This needs to be looked into sooner than later. This is not to alarm you. It is a frank opinion of mine based on experience, blah, blah. Not a medical doctor, a pca survivor, one of many on this fine forum.

Having a biopsy will really make his PSA go up (Mine jumped from about 8 to 13 in a week or so due to my biopsy)
So make sure the next PSA test is not taken too soon. I think the effects of a biopsy on PSA can last as long as 6 weeks.
