You know I love you. But, if indeed the story is as Prat said, I fully stand behind my comments.
Sending a patient to have blood drawn immediately after giving him an aggressive DRE (one that had Prat sore for about
a week) can only be one of three things: incompetence, fraud, or trolling for customers. Can you think of another reason? Especially after Prat mentioned his reservation to the doc and he said they "don't consider it significant". There is ample evidence it is extremely significant!
While I was on my rant I found even more evidence that it is significant. You can look too. Did you see the case where the guy engaged in anal sex the evening before his test and it moved his PSA to 6.5, (6.7?) returning to his normal level of ~1.4 after a retest. Assuming the half life of PSA total is typically 2 days, that means his PSA 30 minutes after the tryst would be up above 8.5! Would a responsible doc send that guy to have his PSA drawn if he knew he engaged in that activity immediately before the draw? O fcourse not. What good is the reading when it can be 5x the true value? Are you going to use that result as a diagnostic aid? It is meaningless. In the instant case, the doc did not not know about
his patient's proclivity for that particular activity. It was after the test was repeated 4 times, twice after activity and twice before activity( results were ~6.5,~1.5,~6.5,~1.5) that the doctor and patient recognized that was an issue - to the great relief of the patient.
Even if the doc is from the dark ages, and is using stone knives and bearskins* to determine PSA, he should err on the safe side and have the blood drawn
before the DRE - especially if the patient... the customer... the one who's diagnosis depends upon good results... requests it.
For that reason alone, I'd move. If that is how NY Presbyterian's Urology Dept as a whole works, I see a huge warning flag.
