Prostate Cancer which has spread to pelvic lymph node

I am very glad you found our little corner of the world here. Great place it is with brothers and sisters (wives, daughters, daughter in law) all who are fighting this prostate cancer. You have been fortunate to already meet a few of our best.

My suggestions;

1. Do NOT goggle anything until you have some idea what is what. Come here and ask questions first. There is so much stuff out on the internet that is either wrong or out of date.

2. Sure get the books. But remember they are probably out of date and other books will contradict them

3. Be calm - Right yeah I know. Scary stuff. There is a lot of information your Hubby needs to get first before he decides on any treatment plan. Other than gathering the information (scans, x-rays, oncologist, urologist) there is nothing he has to do right away. Once he has all the information than he can start deciding on treatment plan.

4. The medical community has a full arsenal of drugs and other treatments - with more coming down the pipe line.

5. When your Hubby does go to the Doc's.
He will want someone with him - preferably someone who can remain calm. Someone suggested audio taping the doctors conversation, that can be very helpful if the Doc agrees.
Have your Hubby out all his questions before he goes to the Doctors appt - than hand a copy of the written questions to the Doc and go over them one by one.

This is a old paper by Dr. Meyers that talks about Hormone Treatments – I’m amazed some of the ‘myths’ he debunks are still out there.

Thanks very much for your reply.  I think I will stop the internet searching now until we have been to the Docs, you are right.  It is very reassuring to know that I now have somewhere to go to talk to people who really do understand.  So many things to think about.

Hi .... my husband was diagnosed in February. He is 64 years old. He also has ulcerative colitis/pouchits and had his colon removed 10 years ago. So his situation is very complicated since he takes immune suppressant drugs. The surgery left so much scar tissue that he is not a candidate for surgery or radiation. His PSA is around 8. His first biopsy showed only 1 core with cancer and gleason of 3+3=6 but the rest of the cores only picked up scar tissue and no prostate cells. We then went to UNC and he had another type of biopsy which showed 3 cores positive out of six with a 3+4=7 gleason. Again, all the doctors have said surgery would be too risky and radiation would have a very negative impact on his UC. The UNC doctor recommended that he go to Duke because they do cryosurgery to at least remove the gleason 7 areas of cancer. He is having a third biopsy at Duke which is an MRI mapping biopsy which will show much more detail of where the cancer actually is. We got the MRI report (biopsy is in two weeks). I read the report and it scares me to death. He has one lymph node which is 6 mm and suspicious of mestastatic disease and a bulge on the right side of the prostate capsule which is consistent with cancer cells escaping. This latest report threw us for a real loop. Every doctor we have seen has told my husband that it is best to do nothing except watchful waiting. Thank God we didn't listen to them. He is going back to tomorrow for a consultation prior to his biopsy. Of course I can't stay off the internet. I've only seen negative things everywhere I've looked about when the cancer has spread to lymph nodes. I know we will hear more details from the doctor tomorrow but I'm terrified of what he will tell us. It sounds like from what I've read on this forum that he will need to go on HT before they can do the cryosurgery. I just want to say that this forum has given me some hope and I feel a little better after reading your posts. Thank you everyone for sharing your stories.

So sorry to hear of your devastating news, but you have come to the right place.  I understand that the MRI found only one spot of PCa outside the prostate located in the lymph nodes, but your consultants are not recommending RT.

In addition to the helpful comments you have received so far, I am offering my two cents.  I am a patient of Dr. Snuffy Myers who put me on to Oligometastases - a complicated term basically meaning that besides PCa in the prostate, it has metastasized to 5 or less areas outside of the prostate. 

Besides Hormonal Therapy, I was treated with SBRT (stereotactic body radiation) targeting the spots on my pelvis as well as my prostate with curative doses.  My story is published in the PAACT magazine  - paactusa.org/uploads/5ed2c3d29_2_paact_summer_2013.pdf.
