can you help me about emerging life saving new treatment for my dad(59yrs).,with
advanced metastatic prostate cancer(left acetablem and pelvic lymph
nodes).,undergone bilateral orchitchtomy a week ago(8jan2010.) in
hyderabad,india---reaccurance of psa after orchitectomy/survival
chances as he has no other health problems/and coming new treatments
to save and prolong life/and method to participate in new treatment al
the way from india.,before going to chemo/---------metastatic advanced
-prostate cancer,spread to pelvic lymph nodes and left acetablem,with
psa 8,and gleasons score3+3=6.,undergone orchyotectomy 10days back in
hyderabad.,no other health problems till now.
.-under gone turp,for bph.,a year back in oct2008.,sadly biopsy sample
was missed by hospital authorities,and urologist failed to estimate
the desease in very frequent visits in DRE.,no new biopsy and psa were
recommended as dr ruled out cancer.,and suspected bph.
-but,now the desesae had been confirmed as metastatic advanced
prostate cancer,spread to bone.,and pelvic lymph nodes.
->psa level is 8.1/
>prostate size is ---53*50*42mm(60cc)heterogenous echotexture,median lobe extending into bladder.
.>trus guided prostate biopsy ,positive for malignancy-gleason's
score6(3+3)-6/6linear cores are infiltrated tumour--focal perineural
spread seen in 1/6cores from the right side.
>mri scan-enlarged prostate with heterogenous signal intensity,focal breach of prostate capsule,involvement of urinary bladder trogone,left obturator lymphadenopathy,left hip joint(actabulum,proximal femur)lesions---rule out prostatic malignancy with secondaries.
>bone scan-left acetabulum finding suspicious for metastasis
----rest no other health problems noted.,and no body and bone pains---

>subcapsular orchitectomy done.,on 8 january2010.
_medication-idrophos50mg,biprosta50mg,b complex and vit-d pills-follow
up after 2 months.

So sorry to hear about your father. I do not have the expertice to give you any advice for tx, but glad you found this site and hope that you find some answers or guidance that can aid your father.

Sorry about your Dad. A gleason 6 that has spread that rapidly is very unusual as the vast majority of gleason 6's are not life threatening. You Dad either has a varient or a much higher gleason. If he were in the US I would recommend he see Dr Snuffy Myers in Earlyville Va. Maybe your dad's doctor could contact him and review his case with him.

TSREDDY. There is incongruity in the diagnostic data which makes it difficult to truly assess the situation of your Dad and come up with advice for proper treatment. With PSA =8 and gleason 3+3 one would not expect metastases and the treatment your Dad received appears to me out of proportion.
Please have the data validated.
Anyway, best wishes for your Dad.

Hello TSREDDY,
At age 58+ and a PSA of 13 ng/ml I was GS5 (3+2) before a radical prostatectomy and upgraded to a GS6 (4+2) after. That said, I had bilateral seminal vesicle invasion and the tumor had invaded the rectal wall but no lymph node or bone mets. After surgery I had an orchiectomy. That was in 1992. My PSA has been <0.2 ng/ml ever since.

A lot depends on your Dad's response to androgen deprivation. This will give you an indication of his prognosis. Stay positive and help your Dad as much as you can. The fact that he is otherwise in good health is very important. Wish you both the very best outcome.

I suppose you could biopsy the the mets and find out the gleason score, not sure that would help at this point. Yes have consults with other oncologist there and abroad. Gleason 6 is a slow growing cancer. Not sure if slow growing necassarily means that it can't metatastizie and quickly. But once it does, does that mean It has to grow quickly, if it is indeed a 6. Any one following my logic here?
Cells, can move quickly thru the blood stream,lymphatic system, but does that mean once they have set up house that the house will not grow slowly. What is your doubling time?, this seems to be the component I would want to know and follow, be for i went into some horrific kinds of treatment that could be overkill. Of course there is the issue of migration and of course there being 1 cell in the biopsy sample that is a 5 own that was not seen. Doubling time, to me, seems to be the Key to this puzzle and the treatments. Chemo at this point? I don't know.
