Just got back from my follow-up visit with Dr. Ash Tewari (LARP doc). Looks like quite good news.
  • Prostate + seminal vesicles combined 32.74 grams
  • Bilateral prostatic adenocarcinoma (predominantly on the left side)
  • Tumor involvement 10% of prostate
  • Staging T2c
  • Gleason Score 4+3. (Biopsy suggested 3+4)
  • Organ confined
  • Negative margins
  • Negative for extracapsular extension
  • Negative for seminal vesicle invasion
  • Positive for perineural invasion
  • Negative for Venous (large vessel) invasion
  • Negative for Lymphatic (small vessel) invasion
  • All lymph nodes (17) clear
  • All nerves spared
I also got the cath removed. Big sigh of relief. Removal was uncomfortable while it was being withdrawn.

So far, no leaking. Only a few extra dribbles after urination. (I drank lots of water - at least four 12oz cups - in the doc's office so he could see that I was able to pass urine and to get an initial view on any incontinence issues.)

Doc said to wait a few weeks before attempting an erection, to give the urethra more time to heal, so no news on ED for awhile.

I have no idea how I would have gotten through this (and will continue to get through this) without my fantastic wife and my brothers/sisters in this group. Thank you all so much!

pretty good pathology other than the gleason upgrade from 3+4 to 4+3. some say it makes a difference, other doctors downplay it. see how your first 2-3 psa tests look after surgery.

sounds good so far on the incontinence part, and as your doctor suggests, too soon to get into the ED side of things, you need good recovery time right now

wonderful news.

bummer on the 3+4 turning to a 4+3 but does seem that you acted soon enough. This is another case that proves how wise it is to seek treatment after a biopsy.

Fantastic news on urinary control. Lately it seems more and more of us are doing great on that front right after surgery.

You've been a model patient. I'm also very impressed with how you've approached your journey.

Other than the Gleason switch it looks great.

Great news. I get blood work done every year because of blood pressure meds and anxiety meds. 2 years ago my PSA was 2.6 went to a urologist  he checked it said will check it again in a year that turned in to 2 went in for blood work seen the report PSA was 3.9. I think its been 9 weeks no pain everything work fine still dribble alittle but thats getting better.

  kind of a funny note to all this was off from work for six weeks because I work in construction went back to work for 2 weeks and got my fingers crushed in a hilo. 2 crushed 1 broken and 2 degloved  24 stitches another week off. Back to work now fingers looking good .Doc said they will be fine in couple more weeks. My luck has got to change.

Gedman,

Good news on the pathology. The upgrade to 4+3  and positive PIN is Ok and probably not much to worry about, but shows you got treatment in time before this could have progressed to a more serious level. Now you just have to sweat out the first couple of PSA tests.

Congratulations on your great report Gedman. I also was impressed with how calmly and rationally you handled your journey. It looks like you will not need biofeedback after all. Hope you continue to heal well.

Thanks for your kind words.

Note that I did two sessions of biofeedback before the surgery. Perhaps that helped me have better post-op continence.

I had just turned 42 at diagnosis with surgery 2 months later. Three kids under 6 and clear of any signs of disease now nearly 18 months later. Seems much the same for you, but with more kids. ;) Congratulations. Listen to your body as you recover. Get stronger.

Update

I feel great. No pain. Walking/sleeping/etc same as I did before LARP. Of course, I'm still avoiding lifting anything heavy.

Although originally I thought that I had no incontinence issues at all, in actuality I do dribble a bit. It seems to happen sometimes when I switch from standing to sitting, and when passing gas. Very tiny amounts. I did a lot of kegel exercises before the surgery, which I will resume today.

Gedman.

The leaks when passing gas is very common as the same pelvic muscles are used to control both the anal and urethra sphincters. It took me months to control this and is still a problem if my bladder is full. The sitting to standing leaks get better as you will need think about  clinching the muscles at first and over time it will become more automatic. Also you can over do the Kegels so only do as many sets as your URO told you to do. After a few months I discontinued doing them altogether.

Gedman, good to hear of your progress!

Just curious....I thought you may have mentioned it in a past post, but couldn't find it...how did you happen to have a PSA test at sch a young age? Family history?

brooklyn: At my annual check-up in January, I mentioned to my doc that I had recently began waking once per night to urinate. He decided to do a DRE (negative) and a PSA test (8.3). Based on the high PSA, I went to the Uro who did a biopsy and found the PCa.

My Uro and Dr. Tewari said that PSA testing is totally outside of standard medical protocol when investigating a 41yo with very minor nocturia (nighttime urination). However, we are all thankful that my doc did the PSA test, because it led to the discovery of my PCa.

Hey Gedman - congrats on your excellent recovery. It sounds like you've been a model patient, and I know that must be a big factor in terms of how you're doing post-surgery. I told my doc I was going to be "the best patient he ever had" - in fact, having read about kegel exercises when I began researching PCa (back in March, when my Uro first told me I needed a biopsy), I began doing them immediately - so by the time I get I to surgery, I'll have a few months of kegels under my belt (so to speak).

Anyway, all that to say that your experiences (and those of the other heroes on this forum) are of great inspiration to those of us that are about to follow in your footsteps. Wishing you continued success in your recovery!
