hi folks,been a while since I've posted but I log on every day to see what's going on in our world.Need some decision making advice here and not sure which way to turn.I made the choice to take a break from ADt some 8 months ago after 2 years on zoladex.At this point my PSA is 71 and rising .....40 two months prior.I couldn't tolerate the dizziness that I got as a side effect of ADt,and also depression.The dizzy spells have lessend after 8 off of ADt but my lymph nodes in the groin area have swollen and are now causing me discomfort.What are my options at this point.?? I'm in the Uk and the Docs have a procedure for metastatic desease.....ADt....chemo...and then as a refractory treatment,Zytiga ( aberiterone).I'm fairly well at the moment considering my stats but would like any views on what can be done for this annoying node as it is affecting my ability to excersise Ect.Also id like to know if the enlarged nodes are indicating that my bone mets have worsened or is it possible that only the nodes have swollen ??.Is a swollen node a tumour in it's own rights or is it just retained fluid ??.I know these are questions for the professionals but it's a slow process over here getting to talk to anyone so I'm asking for advice or info from anyone who might be able to give it.

Not too much knowledge on this, but to enough recommend back on HT after seeing maybe a new oncologist for a fresh opinion.

Hi Lee, I think that a PSA of 71 would warrant starting some sort of therapy soon. Having said this, of course, the doctor should treat a patient, not a number, so depends on you and your doctor. You can go back to ADT or perhaps a second-line hormone therapy with ketokonazole. Lymph nodes swell for a number of different reasons throughout one´life, most common cause is infection (viral, bacterial). If the reason is cancer, any sort of hormonal therapy, or even radiation therapy to the affected node that is bothering you will take care of it. Hope your next line of treatment will give you a long, long remission.

Do they offer you a drug other than zoladex? My oncologist (U.S.) seemed to think that there were enough options in the HT area to keep us busy for a few years. I am just starting a forced vacation after my 6-month Lupron shot, as the side effects have been much more than what the uro & onco expected. In June we re-evaluate and punt.

thanks for your replys everyone,142 yes I've been offered mono therapy with cosadex but found this created bone pain in my lower back where I have mets so I decided not to use that option.I'd also read that some anti androgens such as cosadex ( bicalutamide) are known to fuel certain strains of the desease.On three occasions I've returned to cosadex and experienced pain so I've decided against that.I have substituted my pain killers (ibrufrofen) for another drug celebrex,a cox 2 inhibitor generally used to treat arthritis as I've heard that it shows anti tumour properties in some trials.
