testicle pain- no orgasm

Hi all--- back after a while with new problem... had surgery in Oct-- was able to get erection and have orgasm with cialis after a couple of months, but always had pain in right testicle and in area attaching to it-- told surgeon, he said it was ok, and just healing. Anyway, for the last 5 months, I can get erection, but cannot reach orgasm, and have the same pain, just stronger... I see Surgeon in 2 weeks for follow-up PSA, etc.. any thoughts?

Unless the testicular pain is bad enough to prevent you from climaxing all by itself I suspect that your two problems aren't related. Are you taking any meds that might cause your problem? Antidepressants are a frequent suspect, for instance.
Talk to your doctor about
your pain and go over your drugs with him to see if any of them could be causing your sexual problem.

thanks I will get into the graphic detail when I see him on the 7th--- in short, there is pain in building toward climax, then lack of sensation, and its over-- pain remains for a few hours--
never, ever experienced this before surgery---

You might want to ask your doctor if you have "pudendal neuropathy." (I always find that speaking "Medicalese" gets their attention.) The pudendal nerve goes to your testicles and it is entirely possible that the nerve was stretched or scraped during surgery. You can ask him if he thinks neurontin or an alpha-blocker may help. If it is preventing orgasms, he can laparoscopically disconnect that nerve.
Pain during orgasm (dysorgasmia) after a prostatectomy is surprisingly common, occurring in one in seven patients, according to one study. Complete loss of orgasm (anorgasmia) occurred in 37%. If that seems to be occurring, you might want to talk to a specialist in Sexual Medicine and not just a surgeon.

An anti-depressant, such as Lexapro, can have an unpleasant side effect of causing either very delayed orgasms, or none at all. Let's you function, but after endless amount of effort, you simply won't can't climax. I know this first hand from years ago when I was on it. It was very frustrating. Other meds could have the same effect.
The pain part sounds like a separate issue, it's possible they could scan the afflicted area to make sure there isn't something going on.
Hopefully you will get an answer soon from your own doctor.

Sorry you're experiencing these problems, Riggy. One of the risks/side effects for having a vasectomy is swelling and discomfort in the scrotal area right after surgery. This is caused by build up of fluid in the scrotum. In most men this discomfort goes away with time. In rare cases it persists and leads to epididymitis, a chronic inflammation of the epididymis.
The vas deferens transports sperm from the testicles to the prostate to be mixed with fluids there. With a radical prostatectomy, that passageway has been cut and tied off, so essentially you've had a vasectomy as a bonus procedure.
I hope the orgasms return after the pain issue is resolved. There may be other reasons for your pain, but only your doctor can give you the correct diagnosis.

I have just the pain part. It's mild in my case. A urologist (not the surgeon) suggested it was from things gathering for the release that never happens. He didn't seem concerned about
it and I got the impression it wasn't something he'd been asked about
before.
I agree the anorgasmia sounds like a separate problem, and meds are usually the first suspect. Hope you can get to the bottom of that!
