I poked at their web site and found one direct reference that gave the prerequisites. That one mentioned that it had to be pre-approved, you had to start with Cialis before using the others, and that you had to have had surgery in the urinary tract, amoung others.
But that was all for holders of New York policies that had "complete" coverage.
So it looks like the urinary tract surgery should be met, if you have a policy that covers ED meds. You would need to talk to the insurance company first to see if ED meds are even allowed under the policy you have, and then what the qualifications are.
I had to work through a couple of denials from my (big national) insurance to get 8 pills a month covered. My doctor had to explain that the ED was caused by the DaVinci surgery, which was done under the same policy.
