Surgery was March 26, 2013, and everything is good so far, except ED, which was expected.

So, I got my urologist to prescribe Trimix for me two weeks ago at my appointment with him. My urologist also made another appointment with me for about two weeks later, and told me at that time he would inject me the first time. I took the prescription to the compounding service and went home with it.

Well, I couldn't wait, and have injected 4 times already. I'll go in Monday for my appointment and see what he says about the results so far. I know it was a bit risky, but I started low, after much reading up on it.

1st time: Tuesday .003    .03cc (3 units)    5% erection, no pain
2nd time: Saturday .005   .05cc (5 units)    20% erection, no pain
3rd time: Tuesday   .010   .10cc (10 units)  90% erection, lots of pain, both subsided in one hour
4th time: Saturday   .008   .08cc (8 units)   80% erection, moderate pain, both subsided in 30 minutes

The two erections were enough to be useful, but the pain, especially the first pain, was close to being unbearable for about 15 minutes.

My question is about the formula; the pharmacist at the compounding service told me that he makes 3 strengths, and mine was the lowest.

This is the information from the bottle:

25 mcg alprostadil: papaverine 18.75mg: phentolamine .625mg

What is the likely suggestion I would expect the urologist to prescribe to alleviate the pain, but still provide for the useful erections?

Years ago, after suffering with pain  caused by the Alprostadil (caverject) ,   I learned about BIMIX on this forum. I introduced it to my urologist.  He prescribed bimix and I have been using it successfully ever since.  Nothing else works for me and I have never had pain.  If you get good results with bimix, I suggest using it.

When you tell the uro you went ahead and tried it before your "training" appointment, I expect you will be finding chunks of what used to be your arse all over the walls.

But you'll have to fess up, and he may need to change the prescription. If your insurance is like mine, you'll have to wait a month to get a new batch.

No doubt about it. If you have too much pain; get BiMix from the same compounding pharmacy with a new prescription. It lacks the third ingredient of TriMix (alprostadil) that causes the pain. I used it for 6 months and never got ANY pain. Cost was about $3.00 per injection and it worked just fine. Remember, your healing to get natural erections may take over a year or even 2 years. So don't get discouraged.

Just a note that 10cc second injection of bimix on the left side gave me a much more intense reaction. I thought my member was going to explode, and had some pain associated with that. First injection of 10 cc bimix on the right side was tolerated very well.

A few random reactions, none of them probably that helpful:

Everybody seems to have their units off. The tiniest amount you can draw with a 50 unit syringe -- pulling the plunger out to the first line marked on the barrel -- is 0.01 ml (more than 3 times the 0.003 figure.) On the other hand, you would have to fill the same syringe twenty times to get Steve N's 10cc. You are all talking about insulin syringe "units" which are 0.01ml = 0.01cc = not very much.

As well as trying bimix there are some guys who have an anesthetic added to their trimix as a fourth ingredient. My uro prescribed it for me once but, as it turns out, my compounding pharmacy couldn't mix it. There's some sort of tricky buffering you need to do to make the combination stable. Since I am using trimix for rehab I decided to tough it out with the pain and it has diminished considerably with repeated use.

When my uro gave me the first scrip for trimix he go to chatting with my wife (who he knows socially) and forgot to include the paperwork with the instructions, including coming back for my first jab in his office. When I had straightened out the confusion I did eventually go and let him show me how to do it but not before I had made two (reasonably successful) attempts at home first.

Well maybe you won't have to fess up with your Uro ... BUT if he suggests using the 10u or more amount you can argue with him that you don't feel comfortable with that much .. yadda yadda yadda ask him to try 5 or 7units.

But getting down to the issue about the ache (I assume that is the pain you experienced). You did good in asking your Pharmacist about the various strengths -- low, standard and high.

But what you have got:

Is no where near 'low strength' ... unless your Pharmacist is also a masochist too?

Low should be something like: 5mcg PGE-1
Standard should be around 12.5mcg PGE-1
High-Test should be around 25mcg PGE-1.

Often the Papaverine is just set at 30mg and Phentolamine are .5 or 1 or 2mg (and none of those values will contribute to aching but may contribute to extra length of time of erection without generating extra ache).

So you at least have a starting point. To save time in getting a new prescription with say 12.5mcg pge1, 30MG Pap and 1mg phent, when you get your shot at the docs, be SURE TO COMPLAIN about the ache -- even if you are not getting any ache and then ask for a better prescription while you are there. Yes you spent good money already on the current one, but better to get it right earlier than months later.

Also the extra prescription costs nothing for the doc to write so he should agree to give you one 'Just In Case" you need a lesser version. But be careful. My uro did not even know what the exact Trimix amounts were -- he just wrote 'Standard Trimix" on the script and the pharmacy knew what to make. Since your pharmacy has a little crazy view of strengths maybe you can get your doc to spell it out exactly .. the pharmacist might protest but he has to fill it as it is written.
