should i be worried?i am 54

My last PSA test was 12, which doubled from my last one 6 months ago, and i had a biopsy 6 months ago which was negative, the urologist suggested i get tested again the psa in 6 months(APR 2014). no problems going, only get up 1 time in the night.no family history of it my first psa in 2010 was 5 then it went up slightly to 5.5 then down a bit then up to 6.5 then down to 5.8 around a year ago.

Your path sounds similar to my original one.

I had my first biopsy with a PSA of 5.8, and negative DRE, and no symptoms - came back negative

Had 2nd biopsy one year later, PSA now 12.3, negative DRE, no symptoms - negative

Had 3rd biopsy less than 2 months later, PSA now 16.x, negative DRE, no symptoms, biopsy came
back positive, 7 of 7 cores positive, Gleason 7, up to 90% cancer.

Cancer was there all along, first 2 biopsies simply missed the cancer, the fast climbing PSA was the real clue in my case that the cancer was there.

I would want another biopsy in your case, and sooner rather than later

Hmmm. Answer to your question: Yes, I'd worry.

Six months sounds too long to me. Maybe three months. If that PSA jump is confirmed it needs to be explained.

PSA goes up with age and prostate size - if you are an older man with a very large prostate, a PSA of 6 may be reasonable, but a PSA of 12 is not. I'd be troubled by the sudden rapid rise. It may be prostatitis (it's sometimes symptomless) -- given that your PSA has fluctuated in the past. You can discuss with your doctor doing a 10-day course of Cipro and re-testing after that. While that won't cure most prostatitis, if it does take your PSA down, you'll know that prostatitis was the cause.

You can also ask your doctor to give you a PCA3 test and a free PSA test. If you can't attribute your high PSA to an enlarged prostate or prostatitis, and those tests don't look good, it may be time for another biopsy.

I also think you need another biopsy soon. My husband's first biopsy showed nothing- PSA 6. His doctor waited a year and his PSA was up to 9. He did another biopsy with 16 cores. This one came back positive with Gl 8 in anterior portion of the prostate only. He told us that the first time he did not biopsy the anterior because only 10-20% of prostate cancers are found that part of the prostate and that protocol didn't usually call for a total biopsy the first time. Needless to say I was furious when I found this out a year later, but we did not know then what we know now. Wish I had found this forum at the beginning. We just trusted that the negative biopsy was great news.

Get a 2nd biopsy.  My PSA was 4.6 and on the first biopsy, 1 of 12 cores was positive for cancer.
