Where is the blood coming from?

I had seed implant June 2011. Until a month ago, I was practically free of side effects. Four weeks ago I had blood and clots in urine without warning. I had ultrasound and cystoscopy which found no problem with my bladder. Urologist and radiation oncologist think the blood and clot are due to radiation damage. Two days after the cystoscopy I had severe case of blood and clot in urine. Went to ER, bladder was irrigated and clots were flushed out. Now I am not peeing blood and clots. But if I strain during bowel movements, a few drops of blood appear from the penis. My question is where are these drops of blood coming from? Is it from the bladder, urethra, or the prostate.

I have the same symptoms. Sometimes the clots are enough to block the urethra and require a catheter. I suspect there could be a number of reasons. Mine was originally blamed on a urinary tract infection; an antibiotic cleared up the infection but the sporadic bleeding continued. The second diagnosis put the blame on tissue sloughing off when the tumor was shrinking. The current diagnosis says it's because the mass has escaped the capsule and invaded the bladder. The scans show obvious invasion of the bladder. (I'm having surgery next week to remove the prostate and bladder.) I haven't had radiation yet so can't speak to that.
In short, I think it's very difficult to diagnose. There's blood and the source can't be found for certain short of
opening you up and seeing what's there. Note: I am not medically trained and this is just a layman's observations and opinion.

I have (per cystoscope) significant RT damage to bladder neck & urethra. They break
open and bleed, especially during a difficult BM. Happens every couple of weeks. Scary, but has become ordinary.

I am sure if my radiation-fried bladder were operational and still connected, it too would be bleeding, or perhaps does. However, there is no way anything can either enter or exit my bladder, so there is no way of knowing what's going on inside it anymore.
Hope your bleeding goes away over time, but if it is radiation damage, like 142 above, it may stay with you for a long time. The inside of an active bladder would be a difficult place for natural healing to take place.
Prior to my Urostomy surgery, we waited for one week shy of a year to allow any natural healing to take place, and it never did, forcing me into a decision for that major surgery to by-pass my bladder.

You should ask your Dr. about
hyperbaric oxygen treatment, insurance pays for it usually, the only problem is finding a place that does it. There are only six institutions that have facilities that I know of across the US, I was lucky enough to be close to one (Virginia Mason in Seattle) treatments last 1 1/2 to 2 hours each and it can take up to 40 treatments to get the bleeding to completely stop but its worth it if it works.
The pure oxygen rejuvenates the tissue in the uretha and bladder over time. Your only other option would be something known as VY Plasty, this is where they take tissue for instance out of the inside of your cheek and transplant it in place of the bad tissue in your uretha or bladder neck. I understand its a brutal surgery.
Look into the hyperbaric chamber (Google)

The radiation may cause fibrosis/scar tissue in the lower bladder or urethra that sometimes break off when straining or under the urine stream. It will usually resolve on its own if you leave it alone. A cysto can actually make it worse by rubbing it away. My RO tells guys to avoid a cysto if they can.

when I had a flare up of radiation cystitis not one of my doctors even bothered examining me. they told me it's not something they treat unless it gets really bad. they also said it would go away and probably come back. the cystitis cleared up and I'm going on two years without a return.

Mine started at 18 months out from RT, got to be a big issue for 6 moths of so, then calmed down a bit. Comes and goes every few weeks. Has been an issue now for a little over two years.

I am grateful to many fellow prostate cancer survivors with similar experiences for valuable information and advice. Now I know where it is coming from and so will not get panicky when it happens again. I am still waiting for the result of the cytology and hoping it would not bring any bad news. I realized the cystoscopy dislodged some of the radiation damaged tissues. However, the cystoscopy was important to rule out bladder cancer. I was also concerned about
the blood in urine because of my chronic kidney disease which has been very stable over the last 15 years with my creatinine hovering around 2.0
