I vote for a retest.

That's a big jump in one year. Have you been tested for an infection? You might have a case of protists - that could also cause the PSA to rise suddenly. I don't think the meds you listed would raise PSA that much but that's a better question for your doctor (were you taking these meds when your PSA was 2.5?). Don't rush into a biopsy just yet - you might try a course of antibiotics and see if the PSA comes down which would indicate an infection rather than something else.

Remember that all the PSA test does is to show relative prostate activity. An increase could be for any number of reasons - enlarged prostate, infection, recent sexual activity, bike riding, bad day at the lab - prostate cancer is low on the list.

If it were me, I'd want a retest in a couple of months to confirm the result and possibly get a course of cipro just to rule out infection before thinking about biopsy or other tests. But I am not a doctor, just some random guy on the internet.

Do let us know what your urologist says.

and remember, all you know right now is that there is something odd with your PSA test. What it might be needs more investigation.

How's your urine flow? Another possibility may be that your laser TURP is getting clogged and you need retreatment. Increasing BPH and restricted urine flow can both cause your PSA to rise. I like Andrew's suggestion to try some Cipro first too.

I suggest you talk to your doctor about getting a PHI test instead. It includes both PSA and % free PSA and a third biomarker, -2ProPSA, that is unrelated to BPH or prostatitis. It is covered by insurance and he can order it through the following website:

ps: my urine flow is weak during the night, however not bad during waking hours, i will mention this to the urologist. thanks again.
