First , this seems to be a pretty good plan B down the road when all else fails. My questions are: 1. is there still some type of orgasm? 2. Do you just stay erect until the shot wears off? 3. is there a way to find a dosage that will only keep you erect for lets say 30 min or less or is everything much longer? Cant believe I am asking these questions online, but do appreciate your input.

You can have an orgasm even without an erection. So yes, you can have an orgasm after an injection.

2. Do you just stay erect until the shot wears off?

Yes. If it lasts more than 4 hours, go to Emergency. One guy in my support group got an EpiPen from his doctor.

With Trimix, you titrate up. This means your urologist will start you on a low dose to see if you get a satisfactory response. If not, you'll increase the dose until you do. With Trimix, there may also be a problem with soreness from the alprostadil, which is the most potent ingredient in the mix. That can be adjusted too, and some guys use bi-mix, which leaves it out completely.

Your Uro will train you in dosing, how to inject and what to look out for.

Yep. Here you are asking a bunch of strange guys on the Internet about your sex life. Strange disease, prostate cancer. Kinda turns your head around some times.

Here are some answers:

1) the penile injections won't change your orgasms much so I assume you are also asking abut the surgery. After surgery most men can have orgasms which are pleasurable but different than they were before the knife. A few describe them as more intense but more guys say less intense. You won't produce any semen and you won't ejaculate (except some guys get a squirt or two of urine, especially if the surgery leaves them leaky.) But there is something there. It doesn't all end.

2) Penile injections work by relaxing the smooth muscles around the spongy tissues in the corpus cavernosus. This lets your blood pressure fill the tunca albugena (think sausage casing) giving you an erection. When you climax the nerves signal the receptor cells to release adrenaline into your member, this causes the smooth muscles to squeeze the blood out of the erectile tissues ending the erection. Depending on your dose of injection drugs and on the condition of your nerves this will often work even with injections. But don't count on it.

3) If by injections you mean trimix then it's easy to modulate the dose. Not sure about Caverject which, as I understand it comes in pre-loaded syringes.

Hang in there. Talking about this stuff gets easier after a while. It's empowering in a very odd sort of way.

After 6 years of injections here is my input:

The orgasms are stronger to me, more relaxing.

The dosage, well, the doctor is suppossed to give you the right dosage but, well, that didn't happen for me. His first dose, .20 ml was WAY too much, one of those 4+ hour erections with junior packed in ice, painful, yow!

I started back at .05ml and move up .02-.04 ml per shot. Trimix is cheap but very strong. Make sure you get the sudafed behind the pharmacy counter if you have a 3 hour going on 4 hour erection. The orginal sudafed will bring it down......epipen sounds good too ..

The dosage over the past years 6 years varied a lot, maybe because the needle position varied, shoot for 1000 and 200o positions on the penis. Move up and down the penis to prevent scarring. Maybe because my nerves were spared and were recovering the dosage had very different effects. Lots of four hour erections for sure.

Now .10 ml is the magical amount for me. I use a 31 gauge, 5/16 inch diabetic needle. The pain is minimal.  A usable erection occurs for  2+ hours. Any dose less than .10 ml, no erection. Nada. So the answer to your question is no, you can't have a 30 minute erection with trimix, at least I never could figure it out. Sorry. A minimum of 2 hours. Also if you notice any curvature like I have, you might just inject on the other side only. In other words I developed a mild L side curvature, ie Peyronnies so now I only inject on the R side since the penis will curve torwards the side of scarring. Most men don't get curvature but I did.

Finally if all this calculating and second guessing is too much for you, you might consider a penile implant. Everybody who had one said they're great! Injections kill spontaneity, it's like excuse me, let's have sex, wait 5 minutes while I shoot up in the bathroom. This might be good enough for an old married guy like me who has a wife with no sexual desire anyway and I have to entice  her to get it on twice/month this as it good as it gets.  I don't think an implant is worth it to me for sex less than 24 times/year. But if you want more out of life maybe the implant would be better for you and your girlfriend/wife.

Thanks so much guys for your help! You know your first deal is to get the cancer gone and then get through the incontinence. But later on, you would like to know " it" works again in some way shape or form. thanks again and please reply more if you think of other things that will help.

I had my first injection this morning. It was a very odd doctor appointment to say the least.

I can't add much to this discussion, but after 16 months of nothing, I made the appointment last week and "tried" it out today.
Since I was getting 40 to 45% erect with Viagra, the doc started me on 0.2 Bimix. I thought it was going to work, got to around 45% erect in those first ten minutes after the shot, but nope, it faded away to nothing after 20.
He sent me home with 2 shots of 0.4 and 0.6 to try tomorrow and Thursday.

I was really freaked out and pretty nervous this morning (BP was 186/92) my normal BP is 130/80 and I'm sure that played a role in why I got nothing plus the adrenaline was probably pumping pretty crazy too.

All in all, and a lack of better words, I finally grew "some balls", and tried the injections. It's not nearly as bad as it sounds or looks!

I'm one of the unlucky guys with a venous leak. So that means I would have to use lots of bimix or trimix to maintain an erection more than 30 or 40 minutes. I got up to 45 units of bimix3 and the syringe only holds 50 units. Trimix works about the same at 25 units. Doc pointed me to a device designed for this condition ( I forgot the name right now) but it's basically an adjustable rubber ring to constrict the penis near the scrotum. It lets blood in but not out. It's one more thing to fiddle with so again not good for spontaneity. It does help maintain the erection without using lots of drug but mine still goes away pretty soon after orgasm. That's how it was before surgery so nothing new there. I wasn't looking for a two hour erection anyway, and neither was my wife.

Not only with Trimix, but also with Caverject, you can modulate the dose. I know you can get the powder form of Caverject in various dosages, I have seen 10, 20 and 40 MCG (micro grams), not sure if there are others. And I don't know about dosages available with prefilled syringes, because the powder form is all I was offered.

One obvious way is to mix and fill the syringe with the full dose and use less than the full dose, discarding the remainder. And that way could of course work with the prefilled syringe.

My current prescription is 40 MCG vials of Caverject powder. Last time I mixed it, I filled the powder vial with two syringes of the sterile water, mixed, then drew half the mixture (~20 MCG) and used it; saved the other half a few days in the fridge, drew it with a clean fresh syringe and used it. So I got two 20MCG doses from a 40 MCG container. I had also previously tried the full 40 MCG a couple times and 3/4 of it (30 MCG) a couple of times. I use daily Cialis dose with the Caverject, that has seemed to work well for me with the 20 MCG Caverject. When I use the 20 MCG dose and haven't also taken Cialis in the previous two days, I don't get enough of an erection.

I can achieve orgasm without an erection but having an erection or even partial erection makes it easier because it increases the sensitivity in my glans. I'm not aware that it inherently makes the orgasm "better", but it definitely makes it easier to get to orgasm.

    This is what I have to say about injections DANGER CAUTION!

I used trimix 25 times with great success. I was not advised of anything by the pharm person or my Uro. He just showed me how to inject and that was it. First injection I was super rock hard for 5 hours on my way to the hospital when it finally let down, phew that was scary. Apparently the dose was way way to concentrated and was like rocket fuel. The URO cut it down but advised nothing further. I was never advised about PEYRONIES disease either. I was never advised on moving the needle around to avoid any possible scarring. On the 25th injection my penis bent in half when it became erect. I have been advised by DR's that the Peyronies was not caused by the injections but I still do not believe that for a minute. Many people have had no problem with injectables and many think they have developed Peyronies from the injections.

The only thing I am saying to you is take the extra extra time to make sure you fully fully fully understand what is going on with these injection before you start using them to aviod any future damage to your penis. Does anyone really think you can inject an erection into your penis without some kind of side effect? Some men have used trimix for years with success, I have heard from many that the injections caused then to get Peyronies. once you get Peyronies there is really no easy cure at all. So be forewarn because I was not advised at all about the possible side effects of those injections.

The advice I was given by a fellow PC patient was to make this your last resort. But his reason was a little more practical. He said his erections lasted a few hours but his wife only lasted a few minutes. After the deed was done and she was satisfied he spent the next few hours wondering what to do with himself. Food for thought......

Just following BigAl around so we can give the flip-side of the injection scenario as needed. Been doing shots for 3 years now (mostly trimix) and have done 700+ shots by now with no bends or problems. One thing that might have helped me is that I have been using a VED pump for all 5 years now overlapping the last 3 years. There is some evidence that pumping can help with Peyronies anyways ( or traction) so maybe it can also help prevent it?

  That's good news on the 700 successful injections. I wish I could say the same for real.  I think I'm going to stop posting on the injections and let people make there own decisions without my warnings. I just feel everyone should know that they need to use caution with these procedures because no one told me of any consequences.

I recently had my first trimix injection. Doctor gave me instruction on where to inject and advised that it would take about ten minutes to work. Was also informed that the dosage can be adjusted for better results. There is the issue about the four hour erection but mine lasted about an hour. Seemed very natural but my Doc didn't say anything about side effects or Peyroines. Like the others have said, an orgasm is still possible, it's just different. Because there is no prostate and likely no seminal vesicles, there is nothing to produce.
