The "natural history" as it's called, is far from settled science. It seems likely that there are prostate cancer stem cells that give rise to daughter cancer cells of a single or multiple grades, but which grade can change over time, and the daughter cells have the ability to proliferate, and may be able to progress in grade as they do. All the various cell types influence one another and react to their environment. Certain cells may gain the ability to migrate from and thrive outside of the prostate, called epithelial-mesenchymal-transition (EMT). A lot of very complex molecular processes are involved that we are still learning about.
