I had a surprise phone call from my URO this afternoon. My biopsy result is negative in all 12 cores! I was expecting the results next Monday, but URO received them from the lab today and decided to notify me right away.

URO said we will do another PSA every 3 months and keep close watch on things. If there is another spike, he'll want to do a saturation biopsy. He mentioned sampling areas of the prostate that are not normally sampled in the 12 core template, like the transition zone and the top of the prostate.

Although I am quite pleased, I have read about the "false negative" in Dr. Walsh's book and of course will be concerned until I can absolutely know either way.

My prostate size was measured at 78cc which surprised me, so the biopsy really only sampled about 6/1000 of my prostate.

I'm recovering from the biopsy very well.

Congratulations!Your oversized prostate explains a lot. Maybe get on Avodart or Proscar to shrink that puppy.

Train guy,

I would think that your elevated PSA (5.8) might be explained by the size of your Prostate (78cc).

I like your doctors idea of doing a second biopsy and sampling areas not covered by the first biopsy. My second biopsy detected Cancer in areas that are not usually sampled. The URO did this sampling based on a rapid PSA increase to about 7.8. Of course my biopsies were done six years ago, and I would expect that now you might be able to do some non invasive testing (scans) the doctor may be able to use to target the second biopsy.

Hi Train and congrats on the good news. Re the PSA and prostate size, one rule of thumb says that one should expect one point of PSA for every 10 cc of prostate size. That means your "normal" would be closer to 8 and that your current PSA might not be alarming at all given your big walnut. Of course that's just a guideline and not medical science.

I know added tests are sometimes controversial but, if it was me, I would opt for a MP-MRI before undergoing the next biopsy. That test is getting pretty good at identifying areas of higher grades of cancer and anything that shows up could guide the doc re where to take samples.
