Hello everyone,

Searching for some advice on some news we received. Dad has been having alot of back pain. Onco. ordered an MRI and called right away to say the cancer is pressing on the spinal cord and also a vertebrae. He is recommending radiation right away.

Xtandi appears to have failed as PSA is rising (his PSA has never been a good indicator as at Dx was only 9 when his Gleason was 5+4=9)

As a recap Dad's been on Lupron since 2011 (still), Zytiga (worked for 6 months), Clinical trial of Galeteron (didn't work), Taxotere (worked for 12 months), Xtandi (for the past 6 months- not working).

We are thinking about asking about Xofiogo.... any other advice? We were advised Provenge would not be a treatment option. He does have lymph node involvement, does that rule out Xofiogo?

He's a 5+4 Gleason Dx in Dec 2010....

Spinal Cord Compression is a very serious thing. My husband tried the radiation first, but after several treatments it did not work. One day he was paralyzed from the compression. He ended up having surgery on his back to remove the metastasis and vertebrae and now has two 12 inch rods in his back. He then had to go to rehab for three months to learn to walk again. The surgery was repeated a year later because the hardware failed. This was in 2012. He has very advanced prostate cancer you can see the signature below, but is still going strong.

There are so many treatments out there, each man’s case is different. I would continue to do research and ask the doctor’s what they recommend.

LN involvement doesn't rule out Xofigo, but Xofigo only treats bone mets. SBRT to the spine should provide close to immediate relief. You can zap the spinal mets and any others that may be accessible, and then use Xofigo to help reduce the others.

RT to local bone mets is a valid standard of treatment. I would go with my oncologist's advice if suggested.

For clarity, my T3,T4, and T5 are involved, and I am one inch shorter today than I was four years ago. For me it is general bone loss, at this point.

Thank you everyone. He started radiation treatment - doctor thinks 10 treatments will do the trick.

Waiting on CT and bone scan for next treatment options..... his team is thinking Xofigo.... or Jevanta......

He's now complaining of rib pain... :-\ They stopped Xtandi as it wasn't working and PSA is doubling each month.

I agree with radiation - It's worked for me on mets to L2 (so far...)
