Need advice please?

Hi, i have posted before about my dad condition, after ups And downs , this month PSA came at 4,5 it 's a jump from 1,69 in 3 months, he repeated ct scan and MRT that still showed the spot at the lower of the spine.
His oncologist decided that it's time to start the treatment which is :

chemo with docetaxel.
Zometa for 12 months

Is there anything else that should be done?
Any advice please?

What about Xtandi, with or without Zytiga? New data was announced today, from a Phase II trial, showing Xtandi being effective in controlling PC that is metastatic and resistant to hormonal therapy. Here's the link:

It might be an alternative to chemo at this stage, so worth mentioning to your dad's oncologist. This is especially true if he only has one bone met. If he had many mets, then chemo has shown survival advantage.

My dad might be in a similar situation with a few bone mets (waiting for bone scan results....), and our oncologist has suggested Xtandi + Zometa + HT + Metformin, if that is the case.

Just another idea to run by your doctor.

Laila, I am in a trial using enzalutimide (xtandi) for chemo naive hormone resistant patients. I have nly been on it a short while but my PSA went from 14 to 3 in a month. I also remain on Lupron. The SE's have been mild thus far. I have not had the usual fatigue but do have the hot flashes. I feel better on this regimine than I have been in a long while. The lupron alone used to knock me down but does not seem to be having the same effect with the addition of xtandi.

The course selected by your Dad's doctor is a fairly standard treatment following failure of surgery, radiation and initial HT. I am not sure if I saw HT in the summary on your Dad. Lupron might, zytiga, and xtandi have been shown to have some effect on bone mets.

Does not hurt to ask if there are other treatments.

Laila, chemo is the route I would go (will). Some spot radiation on the Mets if req'd. Best of luck!

Laila, sorry for slow response. Provenge is a treatment that works on your immune system. It is very expensive, and I see your dad is not qualified for medicare. I know very little about advanced cancer except from what I have gleaned here, which may or may not be correct. We have a few heavy hitters in that regard, but being human, we/they grow weary at times. Hopefully they will arrive. The best I can do, and I literally mean the best is keep you and my prayers. take care of yourself Laila, you seem to have an excellent medical team, Keep your faith in them, your husband and well….You Know!

Zytiga has worked great fro me, if your insurance covers it. Doxetaxel or taxotere is the apparent next "sequence", though new studies show promising results with some combinations. Looks like you are in good hands. Others will know much more about multiple agent treatments. like Dr Myers advocates. Keep the Faith!! Dan in So Cal
