Quality of Orgasm after SBRT - or any treatment that causes anejaculation?

I am a 54 year old - recently diagnosed w moderate risk (PSA about 20 - gleason scores of 6 and 7 - appears prostate contained). I am planning to be treated via SBRT at UCLA using the Novalis system. One concern I have is qualify of orgasm after SBRT. I understand I will have little to no ejaculate after treatment. Can anyone (who has had this treatment or any treatment that causes anejaculation) tell me how this absence of ejaculate effects quality of orgasm? Being relatively young for PC treatment - I fear losing my sexual potency/enjoyment ...

Welcome to HW, sorry you need to be here.

Sounds like you've made some decisions already. You won't get total ED, but expect some possible problems. The loss of ejaculate hasn't really affected orgasm. I do get some ED from time to time but the pills correct this just fine (I don't need them all the time, just once in awhile).

Sex won't be exactly the same, but it will still be possible and still fun. You'll adjust. I did.

What I heard before treatment is that the orgasms would be different, and possibly better in some ways. And, that seems to be true for me. Different, absolutely. I've found that it takes more direct stimulation to get things going, but the orgasms are more intense, last longer, and feel just awesome. The loss of ejaculate is a non-issue.

Your mileage may vary.

SBRT means the Tall man would know the best answers. After LDR BT, and almost weaning off of Flomax, I am pretty close to pre-treatment feelings with no ED problems YET and more than half of original ejaculate.

Hi Dave-

For about 45 years I associated orgasm with ejaculation, so orgasm without ejaculation took some getting used to. I concentrated on the orgasm rather than the sloppy wet ending, and found that nothing had really changed. A know some lucky guys who had no diminishment of ejaculate after SBRT or brachy, but I wouldn't count on it. I suspect it may be due to anatomical variations like seminal vesicles that are large and avoid some of the dose. I tell guys to take it as a homework assignment to master bate every day during and for many months after treatment to keep the seminal vesicles and everything else working and well-oxygenated. It's possible that ED meds may have some benefit too.

Other than that, erections are as hard as ever.

Please discuss this with your RO, who I assume is Dr. King. He is under the impression that concerns like this aren't very important because his patients don't bring it up. ROs won't research and address this if they aren't convinced that it's a key issue for the patient.

dp, I was treated using SBRT as well at a year younger than you are. Yes, I lost most of my ejaculate, but orgasms are as intense as ever. I have had no issues with ED. Things are "different" (it's hard to explain), but no loss of function or fun. My sex life is as good as it was before treatment, which is to say great.

DP, after surgery, the first few times I "engaged" in dry orgasm, it felt so weird. I remember saying it felt like "unfinished business" but after the first few times and getting used to it, its actually better. Quite a bit better most of the time, actually

I don't think it's better at all.

It's good, and I still enjoy it greatly but I don't think my ending is as satisfying as it was (especially like 25 years ago....  cool )

Sometimes I have some ejaculate and other times none. I'm still on a daily dose of Flomax so we'll see if that changes when I stop.

My procedure was LDR Brachy. Loss of ejaculate was about 75%. Other than less mess, no change in quality of erection or orgasm.

ED is a ******. Having an orgasm with a limp Willy is not really that encouraging; but its an orgasm nonetheless.

My orgasm post RRP, went from

1. None

2. Orgasm with terrible pain (felt like I had a hernia)

3. Orgasm with no Pain

4. Orgasm with some sticky clear stuff (not pee, from my "other" glands)

just enjoy!

My impression from this site is that enjoyment without ejaculate (or very much ejaculate) varies from man to man. I'll tell you this...it's definitely strange not having it--it feels like you're ejaculating, except nothing comes out. FWIW, wives/partners don't seem to mind and many prefer it.

Thanks for the posts/replies regarding quality of orgasm after SBRT or any PC treatment that causes anejaculation... Glad to hear that for most of us orgasm (with little to no ejaculate) is as pleasurable... and in a few cases maybe better... eases my concerns...
