I think a consult between Sloan Kettering and Epstein's lab is a great idea, though I can certainly understand that the outcome might simply be, "Well, we agree to disagree."

In that case, what recourse would there be other than to err on the side of caution and presume Epstein's more aggressive reading to be correct?

I'm thinking another biopsy, this time MRI guided, then have 3 labs review it. Don't know if I can have another biopsy a month after seed implants?

I don't know about the biopsy, George. Seems like it would be hard to sample around 88 seeds.

Are they planning a post-brachy MRI to check for cold spots? I think usually done at 30 days after. That might help to see what else is going on.

So sorry to hear this. I was also upgraded by Epstein, but I went from a Gleason 8 to a Gleason 9, so that's a bit easier to take. You'll see several G9s here (including me) opt for the triple play of brachytherapy, IMRT and HT. Typically that's planned in advance, but I'm wondering if there's any reason that you couldn't still do this following your brachytherapy.

I agree that surgery is out at this stage, but I don't really see why it would be needed now since your brachytherapy should have killed (or is killing) any PC cells within the gland. With a G9, it's more the worry about what might be outside of the prostate and that's where IMRT and HT can come in.

Three labs won't help you - it's not a popularity contest. One is enough if it's the right one with the greatest expertise in the country - just make sure it's Bostwick or Epstein.

But you can't have a biopsy now - you just had implants and the tissues are all deformed. Remember that radiation kills the cancer over time - so finding it when it may or may not be dead tells you nothing. You can ask your RO about a boost to the known G9 area. A boost like that has been used in a few trials. Lacking that, I think you just have to wait it out and see what happens to your PSA over the years.

My gratitude gentlemen. I like the idea of a boast for the specific area and will raise it with the Docs... somehow I'm hoping this is a typo or some type of mistake. The fact that their are "nines" here is very comforting. God Bless.. will post updates.. We are all in this together.

UPDATE; Appearently my slides.. without informing me,...were also sent to White Plains Hospital. This caused the delay in the first Lab sending my slides to Sloan. However when I phoned WP this morning, their Pathlogy, Radialogy, and Medicial Records Department had no record of anything. It is one core which is positive..that has been rated differently 4 times. My questions.. is it possiblie or even of beniifit to do another biospy? Will doing so put me at risk? Can all these folks decide on a bottom lin e Gleason score? If higher than previously thought.. can they radiate that area and the surrounding area? Bottom line.. Sloan will have the slides and previuos report tomorow morning. My thanks to each of you for your prayers and support.

The best email of my life.." have just spoken with the pathologist. The consensus of the group was that there was not a 5 component and the Gleason score remains 7" MSK

Good news for sure George, very happy for you.

I had a 3+5 that was downgraded to 3+3 by Epstein at Johns Hopkins, so i know how you feel. And at the time, i didnt even realize the repurcussions of having a grade 5 in there, i wasn't sure if it was better or worse than 4+4.

So now you can move forward with some level of peace of mind.

For sure, this "experience" can drain you physically and emotionally. But as you know, or will soon find out, things do calm down and you settle in, get treatment, and will move on to enjoy each day

FIRST PSA BLOOD TEST since seed procedure last July..THANK YOU GOD!!! It's 1.43. Back in April before procedure it was 4.7. It was expected to go down 50%...this is better than lotto. I'm extremely gratefull. God Bless all in this fight

She is a young woman with a bright future along being an exceptional leader in her field. Perfect bedside manor, responsive to pateint needs,and a rare combination of being a nice person and very very smart.I can't recomend her highly enough.
