Avodart to arrest pc growth-20 months update.

Age now 61, dx in 2010, psa 8.37, two cores positive gleason 3+4=7. Robotic in April 2010-PT3A- Gl 3+4=7 perinural invasion and extraprostatic extension identified, rest all clear. Psa undetactable for two years then started upward and reached 0.151 in next two years. Planned for SRT+ADT but dropped the idea at the last moment and started Avodart+Metformine. In about nine months psa came down to 0.091[the lowest] remained around that for 4 months then it started upward and since then it is hovering between 0.120 and 0.150. I am getting monthly psa done. I got a G-68 PSMA Pet CT which gave all clear.

In addition to Avodart and Metformine I spend one hour daily in Gym, am on veg naturopathic diet/life plan, yoga and a positive attitude. Plan is to hold PSA below .2 as long as possible. Hope it works, if not then will think of the next plan. Medical science is striding fast particularly in PC treatment.

I am in a similar situation however will be starting ADT soon followed by IGRT. My psa is just about 0.1 having risen from 0.01 12 months ago (Oct 2014). I had robotic surgery march 2013 and had undetectable psa (less than 0.008) until Oct 2014 when it became 0.01. i am not quite 62 and am seeking the best chance of a "cure' and am willing to risk the side effects.

I have read about the avodart + metformin treatment but wonder if there really is long term data on its effectiveness, plus question whether the lack of a psa rise can mean the cancer is growing but not producing much psa. I recall that saw palmetto can reduce psa level and men are cautioned about using it while monitoring psa, so wonder if the same thing can happen with avodart etc.

Just to tell you that you are doing the best to delay TX. If one is optimistic the PSA elevation is from residual benign tissue and your life-style, monitoring and medications are holding the fort. On the other hand, if PSA is from cancerous tissue then a PSA trigger point SRT/ADT will do the job.The Ga-68 PSMA PET/CT is a good imaging test to have at your corner. Wish you the best possible outcome!

Who suggested you try the Avodart - Metformin drug combo? Did your Oncologist or Urologist write the script for the two drugs based on your wishes or his?

Fairwind it was by Dr Snuffy Myers. He says we will slow/stop the increase of Psa and if not then once the Psa is around 2 go for advanced imaging and nuke where ever it is. Hope it works.

I'm pretty close to the same as you, except I had open surgery, then went to Myers when my psa hit .13 after about 9mo. You can see in my profile it has been as high as 0.199 and as low as 0.124 since, back down to .14 now. He has not verbalized the plan as you stated, maybe he will when I see him next week. He calls it "growth arrest", not AS. At the first visit he said if it worked I would "graduate" in three years. He seems very against radiation in this situation since he wants to avoid the side effects. Yes, I know many men say radiation is no big deal and continence and ED aren't issues. My surgery failed on all three levels in spite of the doc's predictions (though continence is getting pretty good lately). I'm not confident I trust anyone selling radiation or any other treatment as a "no big deal" treatment with "minimal" side effects.
