Lymphocele can form when the lymphovascular vessels are cut during surgery, even without removing the lymph nodes. It is not that rare, happened in about 2-3% of surgery cases. Usually after some time the accumulated fluids will be reabsorbed and the lymphocele goes away. In rare cases they stay inside for a long time. However, as long as it is asymptomatic you should not worry. Things only get interesting when it is infected. What is more worrisome is his PSA rising which indicated a failed surgery.
I had asymptomatic lymphocele after surgery until it was infected 4 years after. It was quite rare, in medical reports I only found 3 cases cited for that to happen so long after surgery.
