I'm curious for example if they can find that small tumor and just cut that off?
That's called "focal therapy," and if you are interested in looking into it, the two leading researchers in the world, Mark Emberton and Hashim Ahmed are at University College London. They don't exactly "cut that off." What they do is use any of a variety of methods (e.g., HIFU, cryo, FLA, PDT, IRE, etc.) to ablate the known tumor.

I'm worrying also that tumor is bigger and just needle spot the tail if I can say that.
Aye, there's the rub. Prostate cancer is multifocal in about
80% of men. That means there are tiny bits of cancer spread throughout the prostate, many too small for detection by even the best imaging/biopsy. The success of focal therapy depends on what's called the "index lesion theory." That theory contends that all the small bits of cancer spread from the main tumor (the index lesion) and the bits it spins off won't then spin off new bits. There's some evidence for the theory, some against. My guess is that it's true for some men but not for others, and we don't yet know how to identify which.
At any rate, since you're fortunate to be in London anyway, I think you should try to get to see Ahmed/Emberton at some point.
