There will continue to be incremental successes and further refinements in imaging and classification, but we are pretty much there TODAY, and have been for 10-years, in terms of successfully identifying AS candidates.
The definition of AS success is NOT that nobody with an initially favorable-risk case dies of PC. Why? The reality is that some men with initially low-risk PC who have sought immediate aggressive treatment have gone on to die from PC anyhow. There simply are some, rare super-aggressive strains of PC which are completely impervious to any treatment. So, the goal of AS is to be as good in terms of PC mortality as the population of men with similar favorable-risk case characteristics who sought immediate aggressive treatment. That's been demonstrated; a good quality AS program is already at least as good as immediate aggressive treatment from a PC-survival standpoint, and better in other QoL aspects.
Nearly 10-years ago, Dr. Peter Carroll summarized in this interview:
It's been our experience when we treat those men that in fact their outcomes appear to be just as good as if we treated them right away. Now the caution here of course is you really want to do your homework and be sure that your cancer is being diagnosed when you've had a very good biopsy, an extended pattern biopsy, you know the grade very well, and the PSAs are generally stable. So you can characterize a group of patients who may be very good candidates for active surveillance versus those who might benefit from more immediate treatment.

1) the treatment modes have incrementally improved since the time of their initial diagnosis for those who seek deferred treatment (that's a "
win"), and
2) many of the men on AS
never need aggressive treatment, and suffer no side effects of standard treatment modes (that's also a "
win").
AS
today is a
win-win situation for well-qualified men (see AS Fundamental #1).
