How long to reach nadir after brachy + radiation

How long will it take to reach nadir after 25 shots of radiation followed by brachy boost?

It all depends on many different factors and can vary considerably. Generally you see estimates in the 18-36 month range. However you have started ADT so that will take your PSA to near zero independent of the radiation. You won't know the effectiveness of the radiation until you have been off the ADT for many months. What length was your last Lupron shot? It has a half life of 6 months so it can take a year or more before you see your T levels rise. What is your T now? What is your PSA?

There is no magic date. Years, but you will reach it. I likely hit mine after 3 years.

My last Lupron shot was December of 2015. I was on it for one year. He wanted to keep me on it but I wanted a holiday. I'll go back on it if necessary. My PSA now is 0.3

I was on HT in conjunction with my brachy/IMRT. When I stopped my HT, my PSA went up but has been pretty steady in the 0.15 to 0.2 range from roughly Month 9 to Month 18. My HT was for 18 months.

Is your PSA up or down? Did you get <.10 while on ADT? How often are you getting a PSA test? You are about 14 months post radiation treatment, so you probably haven't reached a nadir yet from the radiation. You may experience a radiation bounce that often occurs in the 18-36 month range as well. Thus, your PSA may be a bit of a roller coaster over the coming months. ADT can confuse things as well compared to the men that used radiation, but no ADT.

Your dx was a lot like mine and they kept me on ADT for three years. Thankfully I am undetectable six years since my dx. We each had the same treatment except for the length of ADT. I wish you the best and hope you have this beat.

As you can see everyone is different. I immediately went from 40 to .2. It took another four years to go from .2 to .05

My PSA went to <0.1 while on Lupron. Getting tested quarterly. Thanks for the feedback.

I'm working on 5 years and I do not think I am at nadir....at least I hope not.

Just got my testosterone back, it's 238. My Lupron shot was a 3 month and it's been 9 months since I got it so it should be at 50% strength.

I did a forum search on this general topic of "how long to reach nadir after EBT" and found some confusing info.

Understanding that everyone is different, is there a general guideline after each type of treatment? For example, just by the nature of their treatment, the majority of RP guys seems to be there almost immediately. I'm just curious regarding SBRT specifically. Steve's PSA has been on a slow downward slope and is currently .96, but he is only 8-9 months post-treatment. He also started out relatively low with a PSA that bounced around between 2.2 - 2.9 in the 5 months before starting treatment. Both the RO and uro are very vague when we ask questions, but that may be because there is no definitive answer???

Thx in advance for any insights you can share.

Across the population it follows a smooth curve where it drops rapidly at first, but then only declines slowly. For the individual there may be bounces and large variances from the population curve. So you're right that there is no answer. More importantly, the path an individual follows doesn't seem to matter. Slow and bouncier may have a slightly better prognosis, but there are no rules.

Thx you for the response, TA. We always trust what you share with us. I would say Steve's results fall in the "slow" category so far. I think we were both delusional about getting treated, getting cured and going back to our old life. This is definitely the biggest storm we've faced in 45+ years of marriage. We are really struggling with the aftermath.
