I think it is better if a group of doctors evaluate a PSMA PET/CT. You need some experience to do this properly.
At a convention Prof. Amthauer presented the following detection rates depending on the PSA value:
PSA Level 18F-Cholin 68Ga-PSMA
<0.5ng/ml 12.5% 50%
0.5–2.0ng/ml 36% 71%
>2ng/ml 63% 88%
As I said, apparently, if you are on ADT a PSMA PET/CT will work at much lower PSA values.
Some doctors do not think there are many false positives, false negatives are more likely. If you make a LND to take out the lymph nodes which you saw on the PSMA PET/CT, often some still show up again on the next PSMA PET/CT after the LND. The doctor took out different lymph nodes and did not find the right ones.
Dr. Mauer therefore developed the PSMA-radioguided surgery to be able to locate these lymph nodes better:
