Hi Mark, this question used to bug me as well, as my dad was diagnosed with mets with a PSA of 7.5. I used to read a lot and watch many videos, doctors, etc, now I don´t have as much time. At one point, I remember one of the PC oncologists said that the low PSA and extensive disease may mean the aggressive kind of cancer, as people have mentioned, small-cell, etc. Then he said, but it is not always so. There could be low PSA and cancer that is still well-controlled. It turned out true for my dad, who is doing well after almost 6 years. He has adenocarcinoma, the typical histological type. Obviously, it is not giving out much PSA, why so - not sure, we will know more when genotyping of PC becomes a norm some time in the future. All the best.

i know this isn't a recent post, but i wanted to say thanks as this post has given me some much-needed hope today. we will find out on friday if my dad's cancer is anaplastic/small-cell and it's good to see that someone else had low psa/high gleason and it didn't turn out to be one of these other cancers. i am worried sick.
my dad's is a gleason 8 with a psa for 4 at dx, despite a huge tumour mass. that's the highest his psa has ever been!
the doc is concerned as he has one spine met but a very low psa (just over 1 which then went down to 0.45 on its own) but we will know soon enough. i just need some peace and sanity in the meantime which is something i haven't had since this started last month. i know i can't will it away but this has given me some solace today. thank you.
